Object: Explore the expression of ZNF403 and p21 in non-small cell lung cancer(NSCLC)and analyze its correlation with prognosis.Methods: The tissue specimens including 53 patients with advanced NSCLC from June 2013 to February 2016 and 72 patients postoperative tissue are collected from Hunan Cancer Hospital,the clinical data and pathological data of all patients collected were complete,apply immunohistochemistry for detecting the expression of ZNF403 and p21 in 53 advanced NSCLC patients,the above indicators were then further validated in 72 fresh NSCLC patient cancer specimens,finally,analyze its correlation with age,gender,smoking status,clinical TNM stage,histological pathology and survival prognosis.Results:(1)By immunohistochemical staining,the positive expression rate of ZNF403 was 56.6%(30/53)in the inoperable advanced NSCLC,and the positive expression rate of p21 was 58.5%(31/53).The immunohistochemical expression of both had a negative correlation(r =-0.433;p = 0.001).(2)Univariate and multivariate analysis showed that OS in patients with advanced NSCLC was negatively correlated with ZNF403 expression(HR=3.266,p=0.001),and correlated with gender(HR=0.302,p=0.016).(3)The prognosis of ZNF403 negative expression group was longer than that of ZNF403 positive expression group.The median OS was 32.1m vs 18.5m,p=0.007.The prognostic survival of p21 positive expression group was longer than that of p21 negative expression group.The median OS was: 28.4m vs 19.5m,p=0.062.(4)The positive expression rate of ZNF403 in postoperative cancer tissues was 79.2%(57/72),and the positive expression rate in adjacent tissues was 26.4%(19/72).The expression of the two was significantly different(p<0.001);the positive expression rate of p21 in cancer tissues was 55.6%(40/72),and the positive expression rate of adjacent tissues was 73.6%(53/72).There was significant difference between the two(p< 0.001);ZNF403 positive expression is mainly in the cytoplasm,and some in the nucleus,p21 is mainly expressed in the cytoplasm and nucleus;the immunohistochemical expression of the two has a significant negative correlation(r =-0.669;p< 0.001).(5)Univariate and multivariate analysis showed a positive correlation between DFS and p21 expression in patients with postoperative NSCLC(HR=0.375,p=0.034).(6)ZNF403 negative expression group had longer disease-free survival than ZNF403 positive expression group,with median DFS: 34.0m vs 28.5m,p=0.181;p21 positive expression group had disease-free survival than p21 negative expression group.Longer,median DFS were: 32.9m vs 26.7m,p=0.055.Conclusions:1.In the advanced and inoperable locally advanced NSCLC patients,the positive expression of ZNF403 was negatively correlated with OS.2.In the early stage of NSCLC patients,the high expression of ZNF403 was associated with DFS,but there was no statistical difference,univariate and multivariate results showed that the patient's DFS was positively correlated with p21 positive expression,and was not associated with clinical features such as age,gender,smoking history,clinical TNM stage,and histological pathology.
