Background:Lung cancer is the highest incidence and mortality rate of cancer in the world,the incidence is rising year by year,and the five-year survival rate is very low.It is a malignant disease that seriously endangers people's life and health in today's society.Non-small cell lung cancer,which accounts for about 85% of all lung cancer patients,plays a crucial role in lung cancer.TTF-1,CK7 and Ki-67 are commonly used immunohistochemical markers to identify the pathological types of NSCLC and may play a regulatory role in the biological behavior of the tumor.However,the role of TTF-1,CK7 and Ki-67 in the prognosis of NSCLC patients is still not fully understood.Objective:To further explore the role of TTF-1,CK7 and Ki-67 expression in the prognosis of NSCLC patients.We detected the expression of TTF-1,CK7 and Ki-67 in non-small cell lung cancer,and explored their relationship with clinical characteristics and overall survival,so as to provide corresponding guidance for clinical diagnosis and treatment.Methods:1.Pathological specimens of NSCLC patients pathologically diagnosed in Huaihe Hospital of Henan University from January 2013 to December 2018 were collected.Inclusion criteria :(1)NSCLC patients who have completed the corresponding examination to determine the clinical stage before treatment according to the American joint committee on cancer eighth edition(AJCC-8)lung cancer staging criteria;(2)all patients were treated according to the NCCN guidelines for lung cancer;(3)pathological diagnosis was made in Huaihe Hospital of Henan University pathology department,and TTF-1,CK7 and Ki-67 tests were performed;(4)total survival time :More than 3months;(5)obtain informed consent from the patient or family members;(6)approved by the medical ethics committee.Exclusion criteria :(1)patients with metastatic lung cancer or coexistence of other tumors;(2)patients with severe chronic diseases such as heart,brain,liver and kidney;(3)incomplete collection of clinical data;(4)lost to follow-up patients: patients who cannot contact themselves or their family members afterdischarge;(5)patients died of other diseases during the follow-up.2.We collected clinical data of patients meeting the above inclusion criteria,including age,gender,date of diagnosis,smoking history,presence or absence of lymph node metastasis,clinical stage,and expression of TTF-1,CK7,and Ki-67 in immunohistochemical results.The relationship between TTF-1,CK7 and Ki-67 expression and the pathology,stage,smoking,gender,age of NSCLC patients and the overall survival of NSCLC patients was analyzed retrospectively.3.The relationship between the expression of TTF-1,CK7 and Ki-67 and clinical characteristics was evaluated by chi-square test and t-test.The univariate influence of TTF-1,CK7,and Ki-67 expressions on overall survival was evaluated using kaplan-meier method,and the survival curve was plotted.Meanwhile,log-rank test was used for comparison.Results:1.The expression of TTF-1 is related to tissue type,lymph node metastasis and clinical stage,and the positive expression rate is high in patients with lung adenocarcinoma,no lymph node metastasis and early stage.2.The positive expression of TTF-1 was positively correlated with the survival of lung adenocarcinoma in non-small cell lung cancer,and the survival of patients with positive expression of TTF-1 was significantly prolonged compared with those with negative expression.The expression of TTF-1in lung adenocarcinoma can be used as a potential prognostic factor for lung adenocarcinoma.3.The expression of CK7 is related to tissue type,lymph node metastasis,clinical stage and smoking history.The positive expression rate of CK7 is relatively high in patients with lung adenocarcinoma,no lymph node metastasis,early stage and smoking history.4.CK7 cannot predict the prognosis of non-small cell lung cancer and cannot be used as a potential prognostic factor for non-small cell lung cancer.5.The high expression of Ki-67 is related to the pathological stage and lymph node metastasis of non-small cell lung cancer.In patients with lymph node metastasis and later stage,the expression rate of high differentiation is high.6.The high expression of Ki-67 was negatively correlated with the survival of non-small cell lung cancer,and the survival time of patients with high expression of Ki-67 was significantly shorter than that of patients with low differentiated expression,which could be used as a potential prognostic factor for non-small cell lung cancer.Conclusion:1.TTF-1 expression is closely related to clinical characteristics,and its positive expression is positively correlated with overall survival of lung adenocarcinoma,which can be used as a potential prognostic factor for lung adenocarcinoma.2.The expression of CK7 is closely related to clinical characteristics,but positive expression has no correlation with the overall survival of NSCLC,which cannot be considered as a potential prognostic factor for NSCLC.3.Ki-67 expression is closely related to clinical characteristics,and its highly differentiated expression is negatively correlated with the overall survival of NSCLC and can be used as a potential prognostic factor of NSCLC.
