ObjectiveTo explore the factors influencing the prognosis of patients with basilar artery occlusion(Basilar Artery Occlusion,BAO)after endovascular therapy,and to compare the distribution of TCM syndromes before and after endovascular therapy in order to improve the clinical efficacy of integrated traditional Chinese and western medicine in the diagnosis and treatment of basilar artery occlusion.Methods1.From January 2010 to October 2018,we collected the clinical and imaging data of patients with basilar artery occlusion who were admitted to the Encephalopathy Center of Guangdong traditional Chinese Medicine Hospital from January 2010 to October 2018.2.The mRS scores of the patients were collected.The patients were divided into good prognosis group and poor prognosis group with mRS?3 and mRS>3.Relevant data were collected before,during and after intravascular therapy.Renard DWI score,pc-ASPECTS score and DWI BSS score based on DWI sequence on MRI were used to explore the factors related to prognosis after endovascular therapy.3.The distribution of TCM syndromes before and after intravascular treatment of basilar artery was analyzed,and the similarities and differences of TCM syndromes distribution before and after endovascular treatment were compared.4.All the data were analyzed by SPSS 25.0 statistical software.Continuous variables are expressed as meanąstandard deviations,and classified variables are expressed as percentages.The difference was statistically significant(P<0.05).Results1.Logistic regression analysis was used to explore the factors affecting the prognosis of the patients with basilar artery occlusion.The results showed that the Renard DWI score(OR=0.642,95%Cl:0.508-0.810),the time of the onset of the onset of the disease to the inside of the vessel(OR=0.82,95%Cl:023-0.298),the NIHSS score at the time of admission(OR=0.829,95%Cl:0.734-0.937),and the revascularization(OR=3.308,95%Cl:1.380-7.932),etc.The factors were the independent predictors of prognosis(P<0.05).2.the distribution of mental state in patients with basilar artery occlusion was significantly different before and after endovascular treatment(P<0.05).3.The distribution of TCM syndromes in patients with basilar artery occlusion before and after intravascular treatment was significantly different(20.000<0.05).Before intravascular treatment,the TCM syndromes were wind and phlegm stasis.There were 28 cases(20.7%)with obstruction of choroidal obstruction and 37 cases(27.4%)with syndrome of qi deficiency and blood stasis after intravascular treatment.Conclusions1.The patients with basilar artery occlusion had low Renard DWI score before endovascular treatment,short time from onset to receiving intravascular treatment,low NIHSS score on admission,and good vascular recanalization during endovascular treatment.2.Endovascular therapy can improve the mental state of patients with basilar artery occlusion.3.For the patients with acute basilar artery occlusion,the main treatment was to calm the liver and relieve the wind,to remove phlegm and blood stasis,and to treat the patients with basilar artery occlusion in convalescence or sequelae.
