An Observation Of Clinical Effect Of Recuperating Liver And Spleen On Anxiety And Depression After Colorectal Surgery

Objective:To observe the effect of liver and spleen dredging method combined with sertraline hydrochloride on anxiety and depression status and quality of life of patients with colorectal cancer after postoperative chemotherapy,and to explore the advantages and characteristics of combined Chinese and western medicine in the treatment of tumor-related anxiety and depression,so as to provide reference for the treatment of tumor-related anxiety and depression.Methods:From March 2016 to December 2018,119 patients diagnosed as stage ? or III patients with anxiety and depression after colorectal cancer surgery in the outpatient department of oncology and inpatient department of dongguan hospital of traditional Chinese medicine were selected and divided into two groups according to the principle of randomized grouping.The treatment group included 60 patients and the control group 59 patients.The control group was given sertraline hydrochloride 50mg once a day,taking it in the morning.On the basis of the control group,the treatment group was given the prescription of shugan lipi(chai hu shugan powder plus or minus),with 1 dose per day,400ml of water added and boiled into 200ml,and taken warm twice in the morning and evening.The total course of treatment was 8 weeks.Before and after treatment,the two groups were assessed by Hamilton anxiety scale,Hamilton depression scale,TCM symptom scale,quality of life scale,blood routine,liver and kidney function,tumor indicators and other relevant tests,and the data were statistically analyzed.Results:(1)efficacy evaluation of TCM syndromes:after treatment,there was a significant difference in TCM syndromes scores between the two groups(P<0.05).There were significant differences in TCM syndrome score between the two groups before and after treatment(P<0.05).The effective rate(significant+effective)of the treatment group was 93.33%,and the effective rate of the control group was 62.7%,suggesting that the liver and spleen dredging method combined with sertraline hydrochloride can effectively improve the symptoms of "depression" in Chinese medicine.(2)Hamilton anxiety score:after the end of treatment,compared within the group,Hamilton anxiety score of the two groups before and after treatment were significantly different(P<0.05).There were significant differences in Hamilton anxiety scores between the two groups(P<0.05).The effective rate(significant+effective)of the treatment group was 83.33%,and the effective rate of the control group was 57.62%,indicating that the method of liver and spleen dredging combined with sertraline hydrochloride had an obvious advantage in improving the anxiety of patients.(3)Hamilton depression score:after treatment,intra-group comparison showed that Hamilton depression score before and after treatment had significant difference between the two groups(P<0.05).There was no significant difference in Hamilton depression scores between the two groups(P>0.05).There was also no significant difference in the difference of Hamilton depression score between the two groups after treatment(P<0.05).The effective rate(significant+effective)of the treatment group in improving depression status was 76.67%,and that of the control group was 72.88%,suggesting that the treatment method of the treatment group had no significant advantage in improving depression status of patients compared with the control group.(4)quality of life score:after the end of treatment,compared within the group,the quality of life score of patients in both groups was significantly improved(P<0.05).The quality of life score of the treatment group was significantly higher than that of the control group(P<0.05).It is suggested that liver and spleen dredging combined with sertraline hydrochloride is more effective in improving the quality of life of patients.(5)safety analysis:before and after treatment,blood routine,liver function,renal function and tumor indexes were tested in the two groups,and no abnormal changes in liver and kidney function related to taking the study drugs were found in the patients.No adverse reactions related to TCM occurred in the observation group.Conclusion:In the treatment of anxiety and depression associated with colorectal cancer,liver and spleen dredging method combined with sertraline hydrochloride can not only improve the anxiety state of patients,but also reduce the related symptoms and improve the quality of life of patients.Although the effect of improving depression is similar to that of the control group,the overall effective rate is superior to that of the control group.
