Objective: To investigate the incidence of microalbuminuria and its influencing factors in patients with hypertension,and to test the patient's general indicators including UACR(urinary albumin to creatinine ratio),which is a renal function indicator,also to provide experimental evidence for the diagnosis and prevention of early renal impairment in patients with hypertension;further research was conducted to investigate the methods and effects of aerobic exercise intervention in patients with hypertension and early renal impairment.Methods:Part 1:196 patients with hypertension patients from the internal medicine clinic of Sports Hospital General Administration of Sports were collected from March to June 2018.The patients were investigated by questionnaire and general indicators(height,weight,blood pressure,heart rate),UACR-based blood biochemical indicators like blood sugar,blood lipids was detected,and the MAU detection rate was obtained,and UACR value and some clinical indicators for correlation analysis;According to the test results,the patients were divided into MAU negative group(UACR<30mg/g)and MAU positive group(30mg/g<UACR<300mg/g),statistical analysis of clinical indicators of the two groups of patients,conduct comparative analysis will also be done with conducting correlation analysis.Part 2:In this study,MAU positive patients were then screened into the group aerobic exercise intervention was divided randomly into simple hypertensive exercise intervention group and simple hypertensive blank control group,15 persons in each group.The changes of heart rate,blood pressure and UACR value before and after intervention were statistically analyzed,to observe the effect of aerobic exercise on MAU-positive hypertensive patients.Results: 1.The detection rate of MAU was 45.92%.The positive rate of MAU in male hypertension patients was higher than that of female patients;from less than one year to 20 years,the MAU rate of hypertensive patients showed a significant downward trend,from 59.10% to 36.36%,and over 20 years,there is a sudden increase;the MAU positive rate of drinkers is higher than that of non-drinkers.2.The body mass index(BMI)and waist circumference of the MAU-positive group were higher than those of the negative group(P<0.05).The heart rate of the MAU-positive group was lower than that of the negative group(P<0.05).There were no significant differences in age,education level,systolic blood pressure,diastolic blood pressure,pulse pressure,duration of hypertension,duration of antihypertensive medication,salt-eating consumption,and Essen score(P>0.05).3.Correlation analysis results of the detected clinical indicators and UACR values shows that the correlation coefficient between systolic blood pressure and UACR was 0.143(P<0.05),that is,there was a weak positive correlation between the two,and there was no correlation between diastolic blood pressure and pulse pressure and UACR;the correlation coefficient between waist circumference and UACR was 0.213(P <0.05),the two also have a weak positive correlation.4.Considering the influence of the factors on the UACR value,the chi-square test showed that the UACR value of male hypertensive patients was higher than that of female patients(?2=6.506,P=0.011);and according to smoking,drinking,salt-eating consumption,ESSEN score,carotid plaque,blood lipid level,and how long do the patients starts to take medicine after the diagnosis of hypertension,there was no statistically significant difference in the chi-square test results.5.There were no significant differences in heart rate,systolic blood pressure,diastolic blood pressure values before and after the 12 weeks in the two control groups(P>0.05).After having aerobic exercise for 12 weeks,the systolic blood pressure of the patients in the simple hypertension group was reduced by 10 mm Hg(P<0.05).After 12 weeks of aerobic exercise,UACR values were also decreased(P<0.05).Conclusions: 1.In the study,the high detection rate of MAU suggests that for patients with hypertension,monitoring the UACR value is as important as monitoring the blood pressure;the detection rate of MAU in patients with hypertension and diabetes is higher than that in patients with hypertension alone,the detection rate is related to and gender,the duration of medication,and drinking habits.2.UACR is related to waist circumference and BMI level and UACR is positively correlated with systolic blood pressure and waist circumference.It is suggested that positive and comprehensive intervention can control body mass index and blood pressure(especially systolic blood pressure),and it is possible to curb hypertension disease development in microalbuminuria stage,which could avoid complication like further damage to kidney function.3.Aerobics exercise intervention has a positive effect on blood pressure and UACR values in hypertensive patients.Comprehensive intervention is still the core strategy for controlling the condition of hypertensive patients.The importance of non-pharmacological treatment is not allowed to be neglected.
