Objective:To investigate the relationship between gestational hypothyroidism and gestational diabetes mellitus(GDM),and the effects of hypothyroidism and GDM on pregnancy outcome.Methods:From January 2014 to January 2019,87027 pregnant women delivered in Shenzhen Maternal and Child Health Hospital and 83927 Single-birth pregnant women were selected.According to the related test result of thyroid hormone and glucose tolerance test(OGTT)in early pregnancy,5517 pregnant women with hypothyroidism,10326 pregnant women with GDM,798 pregnant women with hypothyroidism and GDM.Then we selected 362 pregnant women in hypothyroidism group,354 pregnant women in normal group and 362 pregnant women in GDM group.Clinical data included age,pregnancy,parity,height,weight,BMI,thyroid function indicators in early pregnancy(including FT4,TSH,TPOAb),OGTT indicators in early pregnancy(including FPG,1hPG,2hPG,HbAlc),and pregnancy outcomes(including macrosomia,pregnancy-induced hypertension,fetal distress,premature delivery,low birth weight infants).SPSS 23.0 software was used for analysis of the relationship between gestational hypothyroidism and GDM was compared and the effects of gestational hypothyroidism,gestational diabetes mellitus and hypothyroidism combined with gestational diabetes mellitus on the pregnancy outcome of pregnant women.Results:1.The prevalence of GDM in pregnant women with hypothyroidism was significantly higher than that with normal thyroid function.2.Hypothyroidism during pregnancy is associated with GDM.3.The rates of hypothyroidism in pregnancy with low birth weight infant and cesarean section were higher than those of hypothyroidism,GDM and normal pregnant women.Conclusion:1)Hypothyroidism in pregnancy is prone to GDM,and there is a certain correlation between them.Hypothyroidism may lead to insulin resistance,thus increasing the incidence of GDM.2)Both hypothyroidism and GDM increase adverse pregnancy outcomes.The incidence of adverse pregnancy outcomes is higher when hypothyroidism is combined with GDM.3)It is suggested that attention should be paid to thyroid function examination of pregnant women.When diagnosed as hypothyroidism,appropriate treatment should be given according to the condition of the diseases to improve pregnancy outcomes.
