Purpose: To analyze the case data of primary SAH patients hospitalized in the People's Hospital of Xiangxi Autonomous Prefecture in recent 5 years,to compare with the literature,to find out the similarities and differences and their causes,in order to improve the level of diagnosis and treatment of SAH.Method: Collect the case data of SAH patients hospitalized in XiangXi Autonomous Prefecture People's Hospital from Jan.2012 to Dec.2017.In statistics,the SPSS22.0 statistical software is applied for the statistics and analysis of the data,and P<0.05 indicates the difference is statistically significant.Results: 120 patients in the group include 36 male patients and 84 female patients,and their average age is 56.54±12.78,in which 94 patients are 40-70 years old and account for 78.33%;But for the intracranial aneurysm,the incidence rate of male and female isn't statistically significant(P=0.080>0.05).Among 120 patients,98 patients suffer from the ruptured intracranial aneurysm;3 patients suffer from the blood coagulation dysfunction disease;1 patient suffers from Moyamoya disease,and 18 patients have unknown causes.19 patients have significantincentives in the onset,most of which is the forcible defecation,the physical activity or rage,etc.105(87.50%)patients have a headache in onset,in which 83 patients have a severe headache(69.17%).74 patients(61.67%)suffer from nausea and vomitting,28 patients(23.33%)suffer from disturbance of consciousness,12 patients(10%)suffer from dizziness,10 patients(8.33%)suffer from syncope,2 patients(1.67%)suffers from epilepticus insultus and 98 patients suffer from meningeal irritation sign.Among 120 patients,112 patients have routine CTA,including 88 positive patients and 24 negative patients,in which the sensibility accounts for 89.58% and the specificity accounts for 87.5%;120 patients have routine DSA,including 99 positive patients and 21 negative patients.111 patients and 9 patients suffer from Craniocerebral positive and negative CT,respectively.All 9 patients underwent lumbar puncture in time.The cerebrospinal fluid pressure increased in 4 cases and normal in 5 cases with onset time of more than 7 days.The cerebrospinal fluid was homogeneous in 6 cases and slightly mixed in 3cases.There were 5 cases of rebleeding,28 cases of cerebral vasospasm,27 cases of hydrocephalus and 15 cases of pulmonary infection.The ruptured aneurysms were predominantly anterior communicating aneurysms(30.6%)and posterior communicating aneurysms(41.8%)in30 cases,and aneurysms less than 10 mm in 96 cases.83 patients were treated with simple coil embolization or stent-assisted embolization.9patients underwent surgical clipping.Glasgow Outcome Scale(G0S)score was 4-5 in 77 patients at discharge.Among 120 cases,91 cases had G0 S score of 5 points,12 cases had G0 S score of 4 points,11 cases had G0 S score of 2-3 points and 6 cases died.Of the 18 cases of PNSH,only one patient had mild disability.The higher the Hunt-Hess grade,the worse the prognosis was(P=0.000 < 0.05).Age leads to poor prognosis.Age and poor prognosis have statistical significance.Spearman grade correlation test shows that the statistic r = 0.195,P = 0.038 < 0.05.The occurrence of complications affected the prognosis with statistical significance,p=0.000<0.05?Conclusion: 1.The clinical features,main causes,imaging examinations and cerebrospinal fluid examinations in this group were consistent with the relevant literature.2.The secondary cause of this group is not arteriovenous malformation,but PNSH.The prognosis and overall prognosis of this group are good,which are significantly different from those reported in relevant literature.
