Objective: To comparatively analyze cardiovascular complications of pregnant women with hypertensive disorder complicating pregnancy(HDCP)between those in elder age and those at the right age.To explore the correlation between age and cardiovascular complications of pregnant women with HDCP,so as to provide guidance for strengthening pregnancy health care.Methods: Retrospective analysis was used in this study.Four hundred and sixty-nine pregnant women with HDCP,who have given birth in First Affiliated Hospital of Hebei North University during the period from January 2016 to June 2018 were selected.According to age,they were divided into the elderly group and the group of the right age.Relevant clinical datas of prenatal pregnant women with HDCP were collected,including age,blood pressure,body mass index,smoking history,blood routine,urine routine,biochemical parameters,thyroid function,HDCP,incidence of cardiovascular complications and related examination results: arrhythmia,ST-T abnormality,myocardial enzymes elevation,heart failure.We compared the two groups of pregnant women by Chi-square test and independent sample T test and we have adopted logical regression model to run the influence factors analysis.Result:1.The AGE,GLU,TC,TG,LDL-C,TSH and CR in elderly HDCP pregnant women were significantly higher than those in HDCP pregnant women of the right age(P<0.05);and HDL-C and FT3 in elderly HDCP pregnant women were significantly lower than those in HDCP pregnantwomen of the right age(P<0.05);there was no significant difference in SBP,DBP,ALT,UA,HB,BMI and FT4 between the two groups(P>0.05).2.Severe preeclampsia was the main type of HDCP in elderly pregnant women,and gestational hypertension was the main type of HDCP in pregnant woman of the right age.There was significant difference in HDCP types between the two groups(P<0.05).3.The incidence of intercurrent atrial premature beat and ventricular premature beat in elderly HDCP pregnant women were significantly higher than those in HDCP pregnant women of the right age(P<0.05);the incidence of intercurrent sinus tachycardia in elderly HDCP pregnant women were significantly lower than that in HDCP pregnant women of the right age(P<0.05);there was no significant difference in the incidence of sinus bradycardia,sinus arrhythmia,paroxysmal supraventricular tachycardia,intraventricular block,atrioventricular block and atrial fibrillation between the two groups(P>0.05).The incidence of intercurrent total arrhythmia in elderly HDCP pregnant women was significantly higher than those in HDCP pregnant women of the right age(P<0.05).4.The incidence of electrocardiographic ST segment depression and ST-T abnormality in elderly HDCP pregnant women was significantly higher than that in HDCP pregnant women of the right age(P<0.05).The electrocardiographic low T wave and myocardial enzyme rising in elderly HDCP pregnant women were higher than those in HDCP pregnant women of the right age,the T wave inversion in elderly HDCP pregnant women was lower than that in HDCP pregnant women of the right age(P>0.05).5.The left ventricular end-diastolic diameter,left atrial anteroposterior diameter and the incidence of heart failure in elderly HDCP pregnant women were significantly higher than those in pregnant women of the right age,the ejection fraction was significantly lower than those in pregnant women of the right age,the difference was statistically significant(P<0.05).There was no significant difference in interventricular septum thickness,anterior and posterior diameter of right ventricle and NT-proBNP between the two groups(P>0.05).The left ventricular end-diastolic diameter and anterior and posterior diameter of right ventricle the incidence of heart failure in elderly HDCP pregnant women were significantly higher than those in pregnant women of the right age,the difference was statistically significant(P<0.05).There was no significant difference in the left atrial anteroposterior diameter,interventricular septum thickness,the ejection fraction and NT-proBNP between the two groups(P>0.05).6.Univariate analysis showed that age,TG,LDL-C and HDL-C had statistical significance(P < 0.05).Multivariate logistic regression analysis showd that the risk factors of HDCP intercurrent cardiovascular disease were as follows: LDL-C(2.166~101.654);TG(1.371~ 2.098)and age(1.020~1.077).Conclusion:1.The levels of blood glucose,blood lipid,TSH and CR in elderly HDCP pregnant women were higher than those in HDCP pregnant women of the right age,while FT3 in elderly HDCP pregnant women were lower than those in HDCP pregnant women of the right age.2.The incidence of HDCP in older pregnant women is higher than that in normal HDCP pregnant women,the type of HDCP is serious.3.The incidence of arrhythmia during pregnancy,ST-T abnormality in ECG and heart failure in elderly HDCP pregnant women was significantly higher than that in the age-appropriate HDCP pregnant women.4.LDL-C,TG and old age are the risk factors of cardiovascular disease during pregnancy in HDCP pregnant women.
