Objective:By analyzing the changes of mean platelet volume(MPV)and maximum platelet aggregation rate(MAR)in plasma of patients with acute atherosclerotic cerebral infarction,this experiment aims to reveal the correlation between MPV,MAR and the severity of acute cerebral infarction.Methods:1.Ninety-nine patients with acute atherosclerotic cerebral infarction admitted to the Department of Neurology of the First Affiliated Hospital of Hebei North University from March 2018 to November 2018 were selected as the experimental group.Neurological defection was evaluated in the experimental group.On the basis of NIHSS score,the patients were divided into light infarction(n=35),moderate infarction(n=36),and severe infarction(n=28).While,the experimental group was divided into large area infarction group(n=31),little area infarction group(n=35)and lacunar infarction group(n=33)on the basis of Adam classification.2.We will choose 68 healthy people as the control group.3.Basic information was collected and blood specimen was drew for routine blood test.The maximum platelet aggregation rate was measured with adenosine diphosphate(ADP)as inducer under PL-12 platelet function analyzer.PC,MPV,PDW and MAR were recorded respectively.4.To explore and analyze the value of blood MPV and MAR in evaluating the condition of acute LAA.Result:1.General data comparison: The data of 99 patients with large atherosclerotic cerebral infarction and 68 healthy people were analyzed in this study.Through t-test and chi-square test,it was found that there were no differences in age(t=-1.128,P=0.261),sex composition(?2=1.791,P=0.181),drinking consumption(?2=1.504,P=0.220),diabetes forbidity(?2=0.109,P=0.741),the number of smokers(?2 = 0.219,P=0.640)and the number of hypertension patients(?2 =0.026,P=0.872)in the two groups.2.Comparisons of platelet-related parameters between the case group and the control group: In the atherosclerotic cerebral infarction group,the PC(t=3.626,P<0.001)was obviously inferior to that in the healthy group,but in MPV(t=-4.263,P<0.001),PDW(t=-2.646,P=0.009),MAR(t=-8.034,P<0.001)were remarkably more than those in the control group.After checking out,P < 0.05,the difference was statistically meaningful.3.Comparison of MPV and MAR in mild,moderate and severe ischemic stroke: In patients with atherosclerotic cerebral infarction,MPV of mild,moderate and severe cerebral infarction were 9.989±0.843,10.614±1.246,11.246±1.506,and MAR were61.283±10.668,77.325±7.712,84.064±8.472,respectively.MPV and MAR increase along with the severity of neurological impairment,and vice versa.After checking out,P < 0.05,the difference was statistically meaningful.4.Comparison of MPV and MAR in lacunar infarction,small area infarction and large area infarction: In patients with atherosclerotic cerebral infarction,MPV of lacunar infarction,small area infarction and large area infarction were 9.988±0.856,10.575±1.257,11.21±1.296,and MAR of them were 60.688±10.578,76.543±7.942,83.896±8.063,respectively.With the increase of infarction area,MPV and MAR increase,and vice versa.After checking out,P<0.05,the difference was statistically meaningful.5.Analysis of the correlation between MPV and MAR: After verification,the correlation between MPV and MAR is significant(r=0.171,P=0.027),and there is a positive correlation between MPV and MAR.Conclusion:1.In the early stage of atherosclerotic cerebral infarction,platelets showed high responsiveness,with the decrease of PC and the obvious increase of MPV,PDW and MAR.2.There is a positive correlation between MPV and MAR.While,MPV and MAR are positively correlated with neurological deficit level and infarct size of stroke.3.MPV and MAR are pathogenic factors for cerebral infarction.They are strongly linked to the severity of cerebral infarction.The combination of the two can be used in clinical work,which has a certain clinical value for disease prediction and the evaluation of cerebral infarction.
