| Objective:To understand the current situation of the patients with severe mental disorders in Hengyang City,and to provide the basis for the government to strengthen the prevention and control of severe mental disorders and develop policies and services.Methods:Data of all patients with severe mental disorders reported in Hengyang City as of June 30,2018 were derived from the National Serious Mental Disorder Data Collection and Analysis System and analyzed statistically.Results:There were 29 539 registered patients with severe mental disorders in Hengyang City,the prevalence rate was 4.10,Qidong County had the highest prevalence rate(4.53),Xiangxiang District had the lowest prevalence rate(2.77);15197 males(51.45%)and 14342 females(48.55%);79.16% of the patients belonged to the labor force aged 18-59 years;the education level of the patients was low,83.26% of the patients were below junior middle school level,and above college level.Only 1.34%;4.30% of the patients with family history of severe mental disorders;40.31% of the patients were unmarried or divorced(37.72% of them were unmarried and 2.59% were divorced);the occupation was mainly farmers,accounting for 69.76%;the poor families were more,accounting for 72.03% of the patients below the local poverty line;91.57% of the patients with six types of severe mental disorders were diagnosed with schizophrenia in management,and only the other five categories were diagnosed.8.43%;Generally speaking,the untreated period of the patients is gradually shortening.Among them,the untreated period of the patients with mental retardation(accompanied by mental disorders)is the longest,and the untreated period of schizophrenia is the shortest.The lowest number of registered patients was in 2010(9 persons),and the highest number of registered patients was in 2014(11061 persons)and 2011(5711 persons).The highest standardized management rate(86.2%)and regular medication rate(80.58%)of registered patients with severe mental disorders from 2014 to 2017 were in 2017.The stable rate of illness showed an increasing trend,while the risk 3 rate showed a downward trend.Patient condition stability rate was positively correlated with standard management rate,medication rate and regular medication rate,and negatively correlated with risk 3 rate.The coverage rate of medical institutions in 12 districts and counties of Hengyang City for the management and treatment of severe mental disorders was 66.67% in Zhuhui District,80% in Xiangxiang District,and 100% in 10 other districts and counties.Women,age < 18,unmarried,low educational level in primary schools and below,poor economic condition,lack of self-awareness,type of disease were schizophrenia and mental retardation(with mental retardation).Delayed treatment is more likely to occur.Conclusion:The group of patients with severe mental disorders has the characteristics of low educational level,high unmarried rate,low economic level,high incidence of treatment delay,long treatment delay and heavy burden on family and society.It is a vulnerable group in society.The government and society should pay close attention to this group.It is very important for the government to manage and treat the patients with severe mental disorders,which obviously affects the stability of patients’ condition and the occurrence of dangerous behaviors.However,there are some deviations in grasping the criteria of identification,diagnosis and classification of diseases in primary medical and health institutions,which affect the quality of management.Therefore,it is necessary to strengthen mental health education,raise the level of social awareness of mental illness,further improve the construction of mental health institutions at the grass-roots level,strengthen the standardized management of severe mental disorders,improve the pertinence and effectiveness of management,and provide a variety of rescue policies and continuous prevention and treatment management strategies to ensure that patients can receive continuous management and treatment services throughout the course of the disease.Promote public safety and social stability,and promote the establishment of a comprehensive Hospital-Community-Family intervention mechanism. |
