Objectives Through the cerebrovascular function detector analysis of the characteristics of cerebrovascular function in patients with diabetes mellitus and homocysteine.Methods The research object selected for annual physical examination at the Medical Center of the Second Affiliated Hospital of Nanhua University from December 2015 to December 2017(Physical examination items of medical examiners include: detection of cerebrovascular function and determination of homocysteinein fasting plasma)And endocrinology department of our hospital diagnosed with diabetes inpatients in the same period(Diabetic patients have undergone cerebrovascular function testing and fasting plasma homocysteine determination).We divided into four groupsAccording to the inpatient case data of endocrinology department and the results of physical examination report collected by the physical examination center of the second affiliated hospital of the Nanhua university:Healthy group(CONTROL group),diabetes mellitus without hyperhomocysteine group(DM group),pure hyperhomocysteine group(HCY group),diabetes mellitus with hyperhomocysteine group(DM+HCY group).were compared the general data of the subjects ?the cerebrovascular functionintegral values ? cerebral blood flow kinematics indexes and cerebral hemodynamics indexes in the four groups;Pearson correlation analysis was performed between plasma homocysteine level and cerebrovascular function score at the same time.Results:1.General data comparison There was no significant difference in age,gender,body mass index,blood pressure and blood lipid between DM+HCY group,DM group,HCY group and CONTROL group(P>0.05)2.Comparison of brain function scores Compared with the CONTROL group,the brain function scores of DM+HCY group,DM group and HCY group were significantly decreased,with statistically significant differences(P < 0.05).Compared with the DM group,the score of brain function in the DM+HCY group decreased significantly,and the difference was statistically significant(P < 0.01).Compared with the HCY group,the score of brain function in the DM+HCY group decreased,and the difference was statistically significant(P < 0.001).Compared with the HCY group,the score of brain function in DM group decreased,and the difference was statistically significant(P <0.01).3.Comparison of kinematics of cerebral blood flow Compared withthe CONTROL group,the HCY group and the DM group,the kinematics indexes Qmean,Vmean and Vmin in the DM+HCY group decreased significantly(P < 0.001).Compared with the CONTROL group,the values of Qmean and Vmin in DM group and HCY group were significantly decreased,with statistically significant differences(P <0.001).Compared with the CONTROL group,Vmean decreased in the HCY group,and the difference was statistically significant(P < 0.05).There was no significant difference in Vmean between DM group and CONTROL group(P > 0.05).Compared with DM group,Qmean,Vmean and Vmin of HCY group decreased,and the difference was statistically significant(P < 0.05).There was no significant difference in Vmax between the four groups(P > 0.05).4.Comparison of cerebral hemodynamics Compared with the CONTROL group,WV,ZCV,RV,DR and CP values of cerebral hemodynamics indexes in DM+HCY group were significantly increased,while DP values were significantly decreased(P < 0.001).Compared with the CONTROL group,WV,ZCV,RV,DR and CP values of DM group and HCY group increased,while DP value decreased,and the difference was statistically significant(P < 0.01).Compared with the HCY group and the HCY group,WV,ZCV,RV,DR and CP values of the DM+HCY group increased and DP values decreased respectively,with statistically significant differences(P < 0.01).Compared with HCY group,WV,ZCV,RV,DR and CP values in DM group were significantly increased,while DP values were significantly decreased(P < 0.01).5.Plasma homocysteine level was negatively correlated with brain function score(Pearson coefficient r=-0.920,P < 0.001)Through bivariate correlation analysis of plasma homocysteine level and brain function integral value.Conclusions:1.Diabetic patients with hyperhomocysteine can significantly aggravate the damage of cerebrovascular function.2.The cerebral vascular function score was negatively correlated with plasma homocysteine level.
