Objective:To observe the effect and safety of recombinant human brain natriuretic peptide(rhBNP) in treating patients with acute myocardial infarction complicated acute left heart failure.Method:Used the contrastive study, we reviewed retrospectively fifty-two patients with acute myocardial infarction complicated acute left heart failure in medical ward of our hospital. Based on different treatment models, patients were divided into two groups, one is the recombinant human brain natriuretic peptide(rhBNP) group for 30 cases(treatment group)and the other is nitroglycerin group for 24 cases (control group). The change of clinical index and curative effect during the treatment, and side effects were observed. All of these patients'vital signs.clinic status and laboratory data were observed at the day before and after the end of the last treatment.Results:1,In two groups, the level of brain natriuretic peptide(BNP). heart rate(HR), respiration rate(RR), SpO2 and urine volume(UV) after treatment were improved. the differences were statistical significance(P<0.05).the liver and kidney function before and after treatment had no statistical difference(P>0.05). We realizde that the vary of HR, UV,BP, RR, SpO2 and BNP in the two groups had no statistical difference before treatmen(P<0.05). Compared with the control group, the treatment group showed a more improve of HR, SpO2 and BNP. and a less decrease of blood pressure (BP) after treatment, the differences were statistical significance(P<0.05).2,Compared with the control group, the treatment group showed a more improve of clinical effect. The efficiency rate of two groups had statistical difference: treatment group [90.00%] vs control group [54.2%]. The day of in hospital of two groups had statistical difference:treatment group [16.1±2.5] vs control group [20.7±3.1].3,In adverse reaction, the patients of two groups had no serious low blood pressure and the liver and kidney function worsening. A few patients of two groups occurred ventricular arrhythmias, but the two groups had no statistical difference(P>0.05). We consider that it is related to the disease itself, not the drugs.Conclusions:The rhBNP can improve heart function of dilatation and contraction and clinical condition with acute myocardial infarction complicated with acute left heart failure obviously, and has reliable and safely effect. The rhBNP is superior to nitroglycerin in treating patients with acute myocardial infarction complicated with acute left heart failure.
