ObjectiveThe aim of this study was to evaluate the safety of taking sini powder by observing the blood tests,and the clinical efficacy of sini powder on indexes of postoperative stress response to laparoscopic hepatectomy by detecting interleukin-6,c-reactive protein and cortisol concentrations.Methods1.Grouping:We selected 46 patients which have accepted laparoscopic hepatectomy in hepatobiliary and pancreatic surgery department of guangdong hospital of traditional Chinese medicine from April 2018 to February 2019.According to the integration and the elimination standard,the samples were divided into therapy group and control group randomly,so that each group was 23 patients.2.Treatment program:After the surgery,the control group was taken conventional western medicine treatment,such as ecg monitoring,eontinuous low-flow oxygen absorption for 24h,nutritional support,adjust water and electrolyte balance,analgesic sedation,protecting liver and stomach and anti-infection treatment.The therapy group was taken the same conventional western medicine treatment as the control group,and decoction of sini powder from the first day after surgery to the fifth day.Sini powder is composed of 15 g bupleurura chinensis,15 g inmature bitter orange,15 g paeony alba and 5 g processed liquorice,and the decoction was prepared by the pharmacy of guangdong hospital of traditional Chinese medicine,1 dose/d,decoction with water,and the course of treatment was 5 days.3.Observation method:evaluating the safety and the clinical efficacy of sini powder by detecting the Liver function,coagulation function,interleukin-6,c-reactive protein and cortisol concentrations before the surgery and on the first day,the third day and the fifth day before and after the operation.Results1.There was no statistically significant differences in gender,age,disease,operative time,intraoperative blood loss and the range of liver segment resection between the two groups(P>0.05).suggesting that the two groups were comparable.2.After the surgery,total bilirubin(TBIL),prothrombin time(PT),interleukin-6(IL-6),c-reactive protein(CRP),and cortisol(COR)were significantly increased and Albumin(ALB)were decreased.ALT,AST,TBIL,PT,IL-6 reached the peak on the first day after the operation and then decreased,while CRP reached the peak on the third day after the operation and then decreased.3.There were no significant differences in ALT,AST,ALB,TBIL,PT between the experimental group and the control group(P>0.05).However,the mean values of TBIL and PT were lower in the experimental group than those in the control group,indicating that sini powder may have a possible tendency to reduce TBIL and PT after laparoscopic hepatectomy.4.There were no significant differences in IL-6?CRP?COR between the experimental group and the control group(P>0.05)·However,the mean values of IL-6 and CRP were lower in the experimental group than those in the control group,suggesting that sini powder may have a possible tendency to reduce IL-6 and CRP after laparoscopic hepatectomy.ConclusionThere was no statistical significance in the effects of sini powder on ALT,AST,ALB,TBIL,PT,IL-6,CRP and COR after laparoscopic hepatectomy.However,from the perspective of mean values,sini powder may have a possible tendency to reduce TBIL,PT,IL-6 and CRP.Sini powder can protect liver function and reduce postoperative stress response after laparoscopic hepatectomy.
