ObjectiveTo investigate the clinical effect of continuous regional arterial infusion(CRAI)combined with Chinese raedicine Sihuangshuimi in the treatment of severe acute pancreatitis(SAP).Methods90 patients with severe acute pancreatitis from December 2011 to December 2018 in DongguanKanghua Hospital were randomly divided into three groups:control group one(n=30),control group two(n = 30)and observation group(n = 30).The three groups adopted common basic treatment methods such as fasting, vital signs monitoring, oxygen inhalation, fluid resuscitation decompression and nutritional support. The different treatment methods of three groups are as follows:the positive drugs such as trypsin inhibitors, low molecular heparin of improving microcirculation and antibiotics control group one was administered intravenously, control group two was administered by continuous regional arterial infusion route, observation group was given traditional Chinese medicine Sihuangshuimi to apply on the abdomen on the basis of continuous regional arterial infusion.Leukocyte count, serum amylase value and APACHE ? score of three groups on admission day, treatment day 6 and treatment day 10 were compared.Besides,The abdominal pain relief time, the first defecation time, the total hospitalization days and the total hospitalization expenses of the three groups also were compared after treatment.ResultsThere was no significant difference in white blood cell count, serum amylase and APACHE ? score among the three groups on the day of admission(P>0.05);the white blood cell count, blood amylase value and APACHE ? score of the three groups on the 6th and 10th day of treatment were Compared. Results show that the observation group was significantly lower than the two control groups(P<0.05);besides,the abdominal pain relief time, first defecation time, total hospitalization days and total hospitalization expenses also were significantly reduced in the observation group compared with the control groups, the difference was significant(P<0.05).ConclusionContinuous regional arterial infusion is superior to conventional systemic intravenous infusion in the treatment of SAP.the clinical efficacy of continuous regional arterial infusion combined with traditional Chinese medicine Sihuangshimi to apply on abdomen in the treatment of severe acute pancreatitis is superior to the conventional systemic intravenous infusion and continuous regional arterial infusion.Three groups were compared show that the clinical symptoms and signs of the observation group recover faster, the hospitalization time of the observation group is shorter, and the hospitalization cost of the observation group is much less. Therefore,it is worthy of clinical application.
