| 1.Research background and purposeAccording to the statistical annual report of global cancer in 2018:in 2018,there will be about 18.1 million new cancer cases and 9.6 million cancer deaths in the world,48.4%of the new 18.1 million cancer cases are in Asia,and 57.3%of the 9.6 million cancer deaths are in Asia.The incidence rate of cancer in China is 23.6%,and the mortality rate of cancer in China is 30%.Lung cancer is still the cancer with the highest morbidity and mortality(morbidity:11.6%and mortality:18.4%).Malignant tumors develop rapidly and have a high mortality rate,which is a serious threat to human health.Although.medical research on tumors has entered the level of small molecules such as genomics and proteomics,there are still no specific drugs to completely treat tumors and improve the prognosis of cancer patients.Current society still is given priority to with chemotherapy for cancer therapy,combined with other auxiliary drugs(such as:5-HT receptor antagonist,hematopoietic cell colony stimulating factor,growth factors,physical therapy,etc.)to improve the curative effect of chemotherapy drugs and reduce the side effects of chemotherapy drug treatment,so as to improve the prognosis of patients with,TCM believes that cancer qi deficiency is due to the human body,unusual,strength variation is caused by endogenous cancer poison;"Vigour" deficient,"defend gas" function is wrong,defend gas cannot seasonable clear is changed"cancer poison",bring about cancer poison to accumulate,hair is cancerous tumour.Literature studies have shown that modern medical immunoregulation is the specific embodiment of traditional Chinese medicine "safeguarding qi",and the application of liujunzi decoction plus or minus combined with conventional chemotherapy in the treatment of non-small cell lung cancer patients with lung qi deficiency syndrome can significantly reduce the side effects of chemotherapy,improve the efficacy,and enhance the patient's immunity.However,due to different clinical reports,the lack of long-term efficacy observation,specific efficacy is difficult to confirm.In this study,the effect of liujunzi decoction plus or minus combined with conventional chemotherapy on specific efficacy and immune function of patients with lung qi deficiency syndrome of non-small cell lung cancer was studied in order to provide evidence for the clinical application of liujunzi decoction in non-small cell lung cancer.2.Research contents and methodsFrom September 2017 to September 2018,a total of 60 patients who were admitted to the department of oncology,zhongshan hospital of guangzhou university of traditional Chinese medicine met the inclusion and exclusion criteria,and were definitely diagnosed as advanced non-small cell lung cancer with lung-qi deficiency syndrome.The patients were randomly divided into the combination group and the control group by computer.The combined group was treated with liujunzi decoction plus or minus combined with conventional chemotherapy.The control group received conventional chemotherapy with western medicine.Before and after treatment,tumor size,improvement of TCM syndromes,improvement of living conditions,tumor markers,immune-related indicators and toxic and side effects of chemotherapy were recorded respectively.In this study,EXCEL was used for data entry,SPSS 25 statistical software package was used for data statistics,n(%)was used for counting data,and fisher test was used for comparison.The measurement data were tested for normality.The data conforming to the normal distribution were represented by s,and the comparison was conducted by t test.P50(P25,P75)was used to represent the data that did not conform to the normal distribution,and mann-whitney U test was used for comparison.In this study,P<0.05 was considered statistically significant.3.Results3.1 RECIST solid tumor evaluation:compared with the control group(46.7%),the objective response rate of RECIST solid tumor in the combined group was improved(55.2%),but the difference was not significant(P>0.05).3.2 subjective curative effect:(40.0%)compared with the control group,the curative effect of TCM syndrome points in the combined group was significantly improved(72.4%),and the difference was statistically significant(P<0.05).(2)the effect of cashew scoring:compared with the control group(43.3%),the significant efficiency of cashew scoring in the combined group was improved(58.60%),and the difference was not statistically significant(P>0.05).3.3 Secondary indicators:_Compared with the control group,CEA decreased more significantly in the combined group after treatment(P<0.01);CA199 decreased more significantly in the combined group after treatment(P<0.01);CD4+decreased significantly in the control group after treatment(P<0.01);CD8+decreased significantly in the control group after treatment(P<0.05).3.4 WHO toxicity grading evaluation:Compared with the control group,the incidence and severity of bone marrow depression in the combined group were significantly lower than those in the control group(P<0.05).Compared with the control group,the incidence and severity of nausea and vomiting in the combined group were significantly lower than those in the control group(P<0.05).Compared with the control group,the incidence and severity of nausea and vomiting in the combined group were significantly lower(There was no significant difference in the incidence and severity of transaminase elevation between the two groups(P>0.05).4.Conclusion4.1 Combined Liujunzi Decoction can significantly reduce CEA and CA199 in patients with non-small cell lung cancer,and the curative effect is better than chemotherapy alone.Compared with chemotherapy alone,CD4+ and CD8+ are significantly lower.Combined with Liujunzi Decoction,it shows certain immunomodulatory ability.4.2 combined with Liujunzi decoction in the treatment of non-small cell lung cancer can significantly reduce the bone marrow suppression and gastrointestinal reactions caused by chemotherapy.4.3 Combined Liujunzi Decoction can significantly improve the TCM symptom scores in patients with non-small cell lung cancer,and the curative effect is better than that of chemotherapy alone(P<0.05).However,in order to improve the living condition of patients(Carlisle score),the combination of Liujunzitang does not show obvious Advantage.4.4 Combined with liujunzi decoction in the treatment of non-small cell lung cancer is similar to that of chemotherapy alone in reducing the size of solid tumor(P>0.05). |
