ObjectiveTo explore the relationship between serum interleukin-6,tissue inhibitor of metalloproteinase-1 and the stability of atherosclerotic plaque in coronary atherosclerotic heart disease and the dialectical relationship between Chinese medicine and cold and heat. This further reveals the relationship between inflammatory response and coronary atherosclerotic plaque stability, and the dialectical relationship between Chinese medicine and cold and heat, thus providing a possible new basis for prevention of coronary heart disease, risk stratification and dialectical of Chinese medicine.MethodsWe chose 96 patients who were admitted to the Department of Cardiology of Shenzhen Traditional Chinese Medicine Hospital from January 2017 to September 20180 The patients who were diagnosed with coronary heart disease were assigned to the control group. The average age(59.69±7.93 years),50 males and 46 females.Among them,33 patients with stable angina(SA),32 patients with unstable angina(UA),and 31 patients with Acute Myocardial Infarction(AMI).At the same time,31 patients who underwent coronary angiography in the same period were randomly selected as the control group. The average age(61.87±9.70 years),including 14 males and 17 females. The general clinical data of all patients' smoking history, diabetes history,TCM syndrome differentiation were recorded. At the same time,3 ml of arterial blood was taken from the test tube before the heparin was given in the coronary angiography, and centrifuged at 3000 rpm for 15 minutes. The supernatant was aspirated and stored in an Ep tube, and stored in a freezer at-80 0 C.The serum concentration of IL-6 and TIMP-1 was measured by ELISA.Coronary angiography results were recorded and values were calculated using the Gensini score. The differences in IL-6,TIMP-1 and Gensini scores between groups and the correlation between IL-6?TIMP-1 and other indicators were compared.Results1?There were no significant differences in the gender, age, smoking history, history of diabetes, history of hypertension,BMI,TC,TG,HDL and LDL between the onservation group and the control group(P>0.05).Among the observation groups, there were no significant differences in gender, age, smoking history, history of diabetes, history of hypertension,BMI,TC,TG,HDL and LDL(P>0.05).2?IL-6 in the observation group was significantly higher than that in the control group(P<0.05).IL-6 levels in the observation group were significantly different among the SA,UA,and AMI groups(P<0.05),and showed the trend of AMI group>UA group>SA group. In the correlation analysis,IL-6 was positively correlated with Gensini score(P<0.01).3?TIMP-1 in the observation group was significantly higher than that in the control group(P<0.05).In the observation group, the serum TIMP-1 levels in the observation group were significantly different between the SA group, the UA group and the AMI group(P<0.05),and showed the trend of AMI group>UA group>SA group. In the correlation analysis,TIMP-1 was positively correlated with Gensini score(P<0.01).4?There was significant difference in IL-6 between heat syndrome group, cold syndrome group and cold-heat syndrome group(P<0.05).Compared with cold syndrome group,IL-6 was significantly increased in heat syndrome group(P<0.05).In the coronary heart disease heat syndrome group, cold syndrome group, cold and heat miscellaneous group. The LDL level of the cold syndrome group was significantly higher than that of the heat syndrome group and the cold and heat miscellaneous group(P<0.05),and the LDL showed the trend of cold syndrome group>cold heat and miscellaneous group>heat syndrome group.Conclusion1?IL-6 levels were significantly elevated in patients with coronary heart disease, and increased in SA,UA,and AMI.IL-6 positively correlates with Gensini score, suggesting that IL-6 not only participates in the development of coronary heart disease. There is also a link between IL-6 and the stability of atherosclerotic plaques.2?TIMP-1 levels were significantly elevated in patients with coronary heart disease, and gradually increased in SA,UA,and AMI patients, and positively correlated with Gensini score. There is a correlation between TIMP-1 and the stability of coronary atherosclerotic plaque, which can indirectly reflect the stability of coronary atherosclerotic plaque.3?In the results of syndrome differentiation of coronary heart disease, the serum IL-6 level in the heat syndrome group was significantly increased.IL-6 showed a trend of heat syndrome group>cold fever and miscellaneous group>cold syndrome group, suggesting that IL-6 level was correlated with cold syndrome of coronary heart disease.4?In the results of syndrome differentiation of coronary heart disease, the serum LDL of the cold syndrome group increased significantly, showing the trend of cold syndrome group>cold fever group>heat syndrome group, suggesting that LDL has a correlation with cold syndrome of coronary heart disease.
