ObjectiveTo observe the clinical efficacy and safety of conventional basic treatment of Western medicine plus three-dimensional traction and simple use of conventional basic treatment of lumbar disc herniation.To explore the role of three-dimensional traction in the treatment of lumbar disc herniation,in order to seek a more effective,safe and simple method for conservative treatment of this disease.To provide a new treatment for lumbar intervertebral disc herniation in the future and to promote it in clinical practice.MethodsBased on the principle of voluntary participation in the study,60 inpatients in orthopedics and traumatology department of our hospital were selected according to the inclusion criteria and exclusion criteria.Using single blind method and simple random method,30 cases were divided into treatment group(conventional western medicine basic treatment plus three-dimensional traction)and control group(conventional western medicine basic treatment),respectively.After two weeks of conventional western medicine basie treatment plus three-dimensional traction and conventional western medicine basic treatment,VAS score of subjective pain,JOA low back pain score,objective lumbar intervertebral disc protrusion index of the two groups were compared before and after treatment.Statistical analysis was carried out on the standard of curative effect of TCM symptoms to evaluate the clinical efficacy and safety.ResultsThere was no significant difference between the two groups(P>0.05)through statistical analysis and comparison of the general data of age,sex and course of disease,which showed that the general data of the two groups were similar and comparable.After 2 weeks of treatment,VAS scores of both groups decreased,JOA scores increased,and the difference was statistically significant(P<0.01),suggesting that the two groups can alleviate pain,alleviate clinical symptoms,improve daily functional activities and improve the quality of life.Inter-group comparison:VAS score and JOA score were compared between the two groups before and after treatment,the difference was not statistically significant(P>0.05).After treatment,VAS score and difference before and after treatment were compared between the two groups,the difference was statistically significant(P<0.01).After treatment,JOA score between the two groups was compared,the difference was statistically significant(P<0.05).Before and after treatment,the difference of JOA score between the two groups was statistically significant(P<0.05).The difference was statistically significant(P<0.01),suggesting that the treatment group outperformed the control group.According to the statistical analysis of lumbar disc herniation index,the lumbar disc herniation index in the treatment group was lower than that in the previous group,with significant difference(P<0.01),while the lumbar disc herniation index in the control group had no significant change(P>0.05).There was no significant difference in lumbar disc herniation index between the two groups before and after treatment(P>0.05);there was significant difference in lumbar disc herniation index between the two groups after treatment(P<0.05).There was significant difference in lumbar disc herniation index between the two groups before and after treatment(P<0.01),indicating that the treatment group increased three-dimensional traction.Treatment can play a role in reimbursement of nucleus pulposus protrusion.At the end of the whole course of treatment,the curative effect was evaluated by comparing the criteria of diagnostic efficacy of TCM syndromes.Statistical data showed that 12 cases were cured in the treatment group,13 cases were markedly effective,4 cases were effective and 1 case was ineffective.The total effective rate was 96.67%.In the control group,6 cases were cured,7 cases were markedly effective,13 cases were effective and 4 cases were ineffective,the total effective rate was 86.67%.There was significant difference between the two groups(P<0.01),indicating that the overall effect of the treatment group was better than that of the control group.No adverse reactions were found in both groups during the whole course of the study.ConclusionConventional western medicine combined with three-dimensional traction for lumbar intervertebral disc herniation and conventional western medicine alone have good therapeutic effect,but conventional western medicine combined with three-dimensional traction can alleviate muscle spasm,correct facet joint disorders,release nerve roots and return prominent nucleus pulposus,and the therapeutic effect is better.At the same time,because the three-dimensional traction therapy is simple,safe,no side effects,and the mechanism is clear,it is worth promoting and applying in clinic.
