Valuation Of Temporary Balloon Intervention Of Internal Iliac Artery In The Treatment Of Placenta Previa

Objective: To explore the effect of balloon intervention of internal iliac artery combined with cesarean section on patients with placenta previa,and to provide clinical data for further promotion of balloon intervention.Methods: 46 cases of placenta previa were retrospectively collected from the obstetrics department of Huainan First People‘s Hospital from July 2014 to October2018.All patients were diagnosed as placenta previa by gynecological ultrasound or magnetic resonance before operation.The diagnosis was also confirmed during operation.All patients were single pregnancy.All patients were divided into two groups according to whether or not they had chosen to implant balloon of internal iliac artery before cesarean section,namely balloon group and control group.The control group was treated with routine cesarean section,and the balloon group was treated with balloon intervention of internal iliac artery combined with cesarean section.The estimated intraoperative blood loss(EBL),calculated blood loss(CBL),intraoperative blood transfusion,hemoglobin change(24 hours before and after operation),Apgar score of newborn birth,hospital stay,hysterectomy and related complications were compared between the two groups.Results: The average intraoperative bleeding balloon group was estimated to be(520.00±342.679)ml,while the control group was(840.97±476.336)ml,the difference was statistically significant(P < 0.05).Before and after operation,the average change of white blood band in balloon group was(12.07±8.803)g/L,while that in control group was(25.61 ±8.496)g/L.The difference between the two groups was statistically significant(P< 0.05).Conversion of blood loss,the balloon group average(742.67±384.624)ml,the control group average(1280.97±537.974)ml,the two groups were compared P < 0.05,the difference was statistically significant;intraoperative blood transfusion,the balloon group average(246.67 ±335.659)ml,the control group average(256.45±346.589)ml,P >0.05,the difference was not statistically significant.he average hospitalization days of placenta previa patients in the balloon group were(8.53± 2.670)days,while those in the control group were(8.00±3.246)days,P > 0.05.There was nosignificant difference between the two groups.The average Apgar score of 1 minute neonates in the control group was(7.90 ±1.680)and in the balloon group was(8.53±0.640).The average Apgar score of newborns in 5 minutes was(9.23 ±0.884)in the control group and(9.60±0.510)in the balloon group.P > 0.05.There was no significant difference in Apgar score of 1 minute and 5 minutes between the two groups.The rate of hysterectomy between the two groups: vs control group(6.7% vs 9.7%),P > 0.05,there was no significant difference between the two groups.Postpartum hemorrhage occurred in 3 patients in the balloon group,and the rate of postpartum hemorrhage was 20%.In the control group,20 patients had postpartum hemorrhage,and the rate of postpartum hemorrhage was 64.5%.Postpartum hemorrhage in the two groups was compared,P <0.05,the difference was statistically significant.Conclusion : Internal iliac artery balloon intervention is safe and effective to reduce bleeding and postpartum hemorrhage during cesarean section in patients with placenta previa,and to reduce the incidence of complications after operation.
