Objective:The aim of this study was to analyze the obstetric outcomes of pernicious placenta previa, to explore the effect on maternal and perinatal, and to formulate the treatment process of pernicious placenta previa, thus provide effective help and effective basis for clinical treatment of pernicious placenta previa.Methods:Data were collected from Women’s Hospital School of Medicine Zhejiang University clinical records on all patients who underwent caesarian section complicated with placenta previa from Jan.2013to Dec.2014. According to the situation whether the placenta overlays the uterine cesarean section scar, patients were divided into pernicious placenta previa group (group A) and non-pernicious placenta previa group (group B). Through statistical comparison between groups of patients with general condition, the types of placenta previa, prenatal vaginal bleeding, intraoperative situation and neonatal outcome.Sum up the impact on maternal and perinatal, and clinical treatment experiences of pernicious placenta previa. Results:During the study of period there were309patients,142in group A and167in group B, who underwent caesarian section complicated with placenta previa, no statistical differences in age, gravidity, parity, abortion and time from previous caesarian operation between the two groups. Group A was significantly associated with higher incidence of placenta accreta, morbidity of postpartum hemorrhage and disseminated intravascular coagulation (DIC), peripartum hysterectomy and the need for blood transfusions than group B(P<0.05). Gestational age, neonatal Apgar score, neonatal birth weight were significantly lower in group A than group B(P<0.05).Conclusions:Pernicious placenta previa complicated with placenta accreta does great harm to the maternal and neonatal, when pregnant with previous cesarean section, attention should be paid during the gestation, especially in late gestation. Appropriate termination of pregnancy should be based on the clinical condition of maternal and perinatal.
