Objective:To explore the correlation between serum 25-hydroxyvitamin D and peripheral vascular disease in type 2 diabetes mellitus,and to provide theoretical basis for the prevention and treatment of peripheral vascular disease in type 2 diabetes mellitus.Methods:one hundred and twenty patients with T2 DM were selceted from July 2018 to March 2019 in the department of Endocrinology and Metabolism in the Second Affiliated Hospital of Nanchang University(63 male,57 female,mean age 55.67 ±12.35years).According to the results of lower limb artery ultrasound and ankle brachial index(ABI),the patients were divided into group with peripheral vascular disease(PVD)and group without peripheral vascular disease(non-PVD).A total of 60 healthy subjects in the physical examination center during the same period were selected as the control group(NC).All subjects' gender,age,course of diabetes,smoking history,and history of hypertension were collected,height and weight were measured,and body mass index(BMI)was calculated.Moreover all subjects were measured fasting plasma glucose(FPG),2-hour postprandial blood glucose(2hPBG),glycosylated hemoglobin(HbA1c),triglycerides(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),serum uric acid(SUA)and homocysteine(Hcy),25-hydroxyvitamin D[25(OH)D].Color doppler ultrasound examination of lower extremity and Limb doppler examination of ankle index were performed by specialists.All data were statistically analyzed using SPSS21.0 software.Results:1.Comparison of clinical data of each groupCompared with the NC group,the FPG,2hPBG,HbA1 c,TC and the prevalence of hypertension were higher in the non-PVD group and the PVD group,while the HDL-C and 25(OH)D were lower(P < 0.05).Compared with the non-PVD group,thecourse of disease,age,smoking rate,BMI,SUA and HbA1 c were higher in the PVD group,while the HDL-C and 25(OH)D were lower.2.Correlation analysis of 25(OH)D level and each indexThe 25(OH)D was negatively correlated with FPG,2hPBG,HbA1 c,SUA and Hcy.3.Relationship between serum 25(OH)D and T2 DM with peripheral vascular diseaseThe proportion of cases in the PVD group with 25(OH)D < 20ng/ml was larger than that in the Non-PVD group,and the difference was statistically significant(P<0.05).Patients in the 25(OH)D<20ng/ml group had a 2.286 times higher risk of T2 DM combined with peripheral vascular diseases than those in the 25(OH)D?20ng/ml group.4.Logistic regression analysis of risk factors for T2 DM complicated with peripheral vascular diseasesLogistic regression analysis was performed to determine whether T2 DM was associated with peripheral vascular disease as a dependent variable,and the course of disease,age,history of hypertension,smoking history,BMI,FPG,2hPBG,HbA1 c,SUA,HDL-C and TC as independent variables.The results showed that smoking history,course of disease,age,HbA1 c and SUA as risk factors for peripheral vascular disease in type 2 diabetic mellitus,and HDL-C,25(OH)D as protective factors.Conclusion:1.In patients with T2 DM,25(OH)D is negatively correlated with FPG,2hPBG and HbA1 c,and the decrease of 25(OH)D is an important factor affecting the occurrence and development of T2 DM.2.Serum 25-hydroxyvitamin D is associated with T2 DM combined with peripheral vascular disease.Serum 25(OH)D level is decreased and the risk of T2 DM combined with peripheral vascular disease is increased.
