Method:According to the literature research of Xinglou Chengqi Decoction for the treatment of stroke,it was estimated that the sample size would include 30 people in each group,and 70 people in total were estimated when the sample shedding rate was considered.Patients with acute stroke due to carotid artery stenosis within 3 days were selected in our hospital.Cerebrovascular stent implantation was performed within 2weeks or 48 hours after onset,and MRI examination and NIHSS score were completed within 24 hours after admission.Patients who do not meet the indications and inclusion criteria of CAS surgery will not be included in the group.Patients will be randomly divided into the treatment group and the control group according to the treatment order and the grouping design plan.Patients who have serious complications or fail to adhere to the treatment during the treatment of the two groups will meet the requirements of 30 or more patients in each group after treatment.The two groups were given basic western medicine treatment and improved perioperative preparation,while the treatment group was given Xinglou Chengqi Decoction orally for 14 days.NIHSS and mRS scores at14 d,30d and 3 months after CAS were observed,NSE levels were detected,and adverse outcomes such as hyperperfusion events and clinical prognosis were observed.To evaluate the effect of Xinglou Chengqi Decoction on the short-term prognosis of acute stroke with CAS carotid artery stenosis in perioperative period.Results: A total of 70 patients were included,7 of whom were separated due to non-adherence to treatment,and randomly divided into 30 patients in the treatment group and 33 patients in the control group.The baseline data of the two groups were not statistically significant.Under different perioperative treatment schemes of CAS,CRP,blood viscosity and LDL-C levels were reduced in the treatment group and the control group two weeks after the perioperative period of CAS,and there were statistically significant differences(p<0.05).After 2 weeks and 1 month after CAS,the node timeneurological deficit score and NSE level were observed,and the treatment group was more satisfied than the control group,with statistical significance(p<0.05).The residual stenosis degree of CAS was different between the two groups.There was no significant difference in NSE level,mRS and NIHSS score between the two groups at 3 months after CAS(p>0.05).There were statistically significant differences between the two groups in hyperperfusion events 2 weeks after CAS(p<0.05),and no statistically significant differences in the time of new cerebral infarction with three nodes.There was no statistically significant difference between the two groups(p>0.05)in the hyperearly and delayed stage.After 3 months of Logistic regression model analysis,it was found that: NSE level at admission(p=0.01,OR:0.84,95%CI:0.74-0.96)and CRP level after treatment(p=0.03,OR:2.55,95%CI: 1.08-6.02)may be key predictors of short-term prognosis,while residual stenosis subtype has no significant influence on short-term prognosis(p>0.05).Conclusion: Xinglou Chengqi Decoction combined with basic Western medicine can reduce blood viscosity,CRP,LDL-C and NSE levels during the perioperative period of CAS for acute ischemic stroke with phlegm-heat-fu syndrome.It can promote the early recovery of neurological function after CAS(1 month),but has no significant effect on the prognosis of short-term nodal time(3 months)after CAS.
