Objective: Primary vaginal cancer is a rare malignancy of the female reproductive tract.Squamous cell carcinoma is the most common pathological type of vaginal cancer, while adenocarcinoma accounts for a small proportion.Intestinal adenocarcinoma among adenocarcinoma types is less common and rarely reported clinically. The purpose of this study is to report a case of primary vaginal villoglandular adenocarcinoma and to analyze the relevant cases. Methods: To report and analyze the diagnosis and treatment of one case of primary vaginal villoglandular adenocarcinoma in the First Affiliated Hospital of Nanchang University in August 2018,and to collect and summarize the cases of primary intestinal adenocarcinoma in recent years.Result: There were 5 cases of operation alone,3 cases of operation combined with radiotherapy, and 1 case of radiotherapy alone and 1 case of radiotherapy combined with chemotherapy.Among the 5 patients who underwent surgical treatment alone,2 patients with stage FIGO II underwent radical surgery, and no recurrence was found in the follow-up of 20 months.3 patients with FIGO I were operated by local resection, partial vulvectomy, anterior pelvic dissection, respectively. These patients were followed up for 6 weeks to 25 months without recurrence.Among the 3 patients treated with surgery combined with radiotherapy, local recurrence was observed in 1 patient with 22 months of follow-up.1 patient was alive with disease during 8 months of follow-up,and 1 patient had no recurrence with 5 years of follow-up.One patient with radiotherapy alone was uncontrolled with 3 months of follow-up.Conclusion: Primary intestinal adenocarcinoma is a rare disease with few clinical data. The main clinical symptoms are abnormal vaginal bleeding and discharge.Metastatic tumors, especially gastrointestinal malignancies, should be excluded. At present, radiotherapy has become the main treatment for primary vaginal cancer. With the improvement of laparoscopic techniques and thedevelopment of vaginal reconstruction techniques in recent years, the advantages of surgery for young patients with earlier clinical stages have gradually emerged.
