ObjectiveInvestigate the injection environment of medical institutions,the safety injection knowledge and behavior status of nursing staff,and initially understand the status of safe injection in China,provide basic data for regulating the injection behavior of nursing staff and promote safe injection,and provide the basis for the formulation of safety injection policy in China.MethodsQuestionnaire survey and on-site observation were used to investigate the status of safe injection in stages.Each medical institution observed 2 injection sites,surveyed 10 nursing staff,and collected and summarized the data for statistical description and analysis.In the first stage,relying on the National Hospital Disinfection and Infection Control Monitoring Project,68 medical institutions in 20 provinces,municipalities and autonomous regions were surveyed on the status of safe injection of medical institutions and nursing personnel.In the second stage,a survey was conducted on the status of safe injection of environmental and nursing staff in 20 community health service centers in Beijing.Result1.Status of safe injection in national medical institutionsA total of 631 nursing staff from 68 medical institutions in 20 provinces,municipalities and autonomous regions were surveyed,and 143 injection sites were observed.The total awareness rate of national nurses' safety injection basic knowledge was 84.54%,and 98.10%of nursing staff could understand the concept of safe injection,but the awareness rate of safe injection range was only 25.36%.Safe injection attitude:97.62%of nursing staff can realize the importance of safe injection,but only 18.20%of nursing staff believe that current injection abuse is more common.The safe injection behavior rate of nursing staff was 86.35%,and the lowly safe injection behavior rate:hand washing between two injections(69.89%),and hand washing before injection operation(72.27%).At present,5.71%of nursing staff can't do "one person,one needle,one tube".The incidence of sharp injuries in nursing staff in the past 3 months was 13.31%,the average annual incidence was 0.44 times/(person,years),and 61.90%of people with sharp injuries did not report to the superior.On-site observations showed that 98.50%of medical institution were adequately inoculated with ambient light,97.20%of medical institution were equipped with sharp boxes,but only 71.3%of medical institution were equipped with dry hand facilities.There are mainly the following unsafe behaviors during the injection process:77.60%of inject site has less than 30 seconds to dry the skin after disinfection,23.80%of inject site has disinfected the skin with a dry cotton swab after disinfection,and 57.34%of inject site has freehand ampoule,20.28%of inject site has manual destruction of syringes and needles.2.Status of safe injection in Beijing's primary medical institutionsA total of 146 nursing staff from 20 community health service centers in Chaoyang District of Beijing were surveyed and 40 injection sites were observed.The total awareness rate of safe injection basic knowledge of Beijing nursing staff was 82.72%.99.32%of nursing staff had heard of safe injection,and 95.21%of nursing staff knew the concept of safe injection,but only 27.4%of nursing staff knew safety.Shooting range.Safety injection attitude:97.26%of nursing staff can recognize the importance of safe injection,but only 17.80%of nursing staff believe that current injection abuse is more common.The safe injection behavior rate of nursing staff is 85.87%.In the process of injection,100%can achieve "one person,one needle and one tube",and 100%of the injection sites are equipped with sharp boxes.Each time check the disinfection and sterilization of the injection device(70.55%),wash hands between two injections or punctures(73.29%),after the skin is disinfected,after the injection(76.03%),disinfection swabs that do not need to be dried after skin disinfection(The cotton swab)wipes the disinfection site(76.03%)and the execution rate is not high.The incidence of sharp injuries in nursing staff in the past 3 months was 10.96%,with an average annual incidence of 0.44 times/(person.year).62.50%of people with sharp injuries did not report to their superiors.On-site observation shows:In the injection site safety injection facilities,the mobile water hand washing facilities and sharp box can be equipped to 100%,97.50%of the site is equipped with quick-drying hand disinfectant,but only 70%of the injection site is equipped with Dry hand facilities.The following unsafe behaviors mainly exist during the injection process:75.00%of the injection site is disinfected and the skin is allowed to dry for less than 30 seconds,40.00%of the injection site has freehand ampoule,and 22.50%of the injection site has manual destruction of syringes and needles,10.00%of the injection site has a double glove back needle cap.3.Differences in safety injection knowledge,attitudes,behaviors and influencing factorsThere were significant differences in the scores of safe injection basic knowledge among nursing staff of different ages and working years(P<0.05).The safety injection behavior scores of nursing staff of different ages,years of work,education,and safety injection training were diferent,and the difference was statistically significant(P<0.05).There was a positive correlation between safe injection attitude and safe injection behavior(?=0.149,P=0.000).Stepwise substitution into multiple linear regression analysis showed that there were 9 statistically significant factors affecting safe injection behavior,such as providing injection and strengthening protection for AIDS patients,working years,undergraduate and above,etc.Conclusion1.Nursing staff have insufficient mastery of safety injection knowledge,lack of awareness of injection abuse,and insufficient safety injection training.Medical institutions should establish a reasonable training cycle and training mode to improve the safety of injection knowledge and attitudes of nursing staff.2.Nursing staffs injection behavior is not standardized,mainly there are problems such as poor compliance of nursing staffs hand hygiene,insufficient action time after skin disinfection,and high incidence of sharp injuries.Medical institutions shall formulate safety injection rules and regulations,improve the safety injection operation process specifications,standardize the safe injection operation of nursing staff,and improve the self-protection awareness of nursing staff.3.Some medical institutions have inadequate safety injection facilities,and auto-disable syringes have not been widely promoted.It is necessary for the state to increase support for the deployment of safe injection products for medical institutions,and to formulate policies and strategies for the promotion of auto-disable syringes to reduce infections caused by unsafe injections and occupational injuries to caregivers.
