Construction And Preliminary Application Of Evaluation Index System Of Nursing Staff' Safety Injection Behavior

Objective To construct a safety injection behavior evaluation system for nursing staff,and to provide a tool for scientifically evaluating safety injection behavior of nursing staff.Using the index system to observe and evaluate the status of safe injection behavior of nursing staff,and explore the weak links in the implementation of safety injection for nursing staff,which provides the basis for clinical managers to improve the normative behavior of safety injection for nursing staff and promote the management of clinical safety injection.Methods Research?:The construction of evaluation index system of safety injection behavior of nursing staff.The study adopted phenomenological research,a research methods of qualitative research.Based on semi-structured interviews of 22 nursing professionals,we collected and analyzed their opinions and suggestions on the evaluation of safety injections of nursing staff.The first round of expert consultation questionnaire was completed on the basis of qualitative interviews and literature review.After that,two rounds of letter consultation were conducted on 18 nursing management experts and nursing education experts from all over the country.The evaluation index system of safety injection behavior of nursing staff was determined.In order to facilitate the application of the index system by managers,a "nursing staff safety injection behavior evaluation form" was developed.Research?:Verify the reliability and validity of "nursing staff safety injection behavior evaluation form" .The structural observation was used to investigate the safety injection behavior of nursing staff in a Three-A hospital and a Two-A hospital in Hefei,Anhui Province,and the results were statistically analyzed.Results Research?:After two rounds of expert consultation,experts'opinions reached consensus.The recovery rate of questionnaire in two rounds was both 100%,and recovery efficiency was 90.00%and 100%,respectively.The authority coefficient in two rounds was 0.923 and 0.919,respectively.The coordination coefficients of first-class,second-class,and third-class indexes(observation points)in the second round were 0.183,0.202,0.278,respectively.Finally,a tertiary structure index system consisting of three dimensions: "safeguarding patient safety" , "safeguarding nurse safety" and "Safeguarding public safety" was formed.It included 3 first-class indexes,7second-class indexes and 21 third-class indexes(observation points).On the basis of the establishment of the index system,a "nursing staff safety injection behavior evaluation form" was formulated.Research?:On the Reliability and Validity Test of "nursing staff safety injection behavior evaluation form" :the logical analysis showed that the content validity of the evaluation form was good,test-retest reliability test results showed that the test-retest reliability of evaluation form was high(r=0.788,P<0.01),inter-rater reliability test results showed that the Kendall'W value is 0.93 and the x~2value is 8.37,all reached significance level.Survey results on safe injection behavior of nursing staff show the total score of safety injection behavior of nursing staff was(34.89±3.30)points.The "safeguarding patient safety" dimension scored(25.59±2.52)points.The "safeguarding nurse safety" dimension scored(5.82±1.73)points.The "safeguarding public safety" dimension scored(3.48±0.59)points.The items with higher scores in the third-level indicators(observation points)are, "?-19 Avoid sharps injuries;if sharps injuries occurs,it shall be disposed according to the process of sharps injuries treatment" (1.99±0.16 points), "?-12 Choose the right injection site" (1.97±0.21 points), "?-9 Correctly check the doctor's advice" (1.97±0.17 points).The items with lower scores are, "?-4 Hand hygiene before preparing liquid medicine and(or)before and after injection" (0.98±0.78 points), "?-10 Check before,during and after the injection:bed number,name,drug name,concentration,dose,time,usage,wristband information" (1.21±0.42 points), "?-17 Avoiding two-handed recapping of needles,and one-handed techniques should be used if recapping is necessary" (1.24±0.95 points).Conclusion 1.Constructed a set of scientific and systematic nursing staff safety injection behavior evaluation index system.Developed a "nursing staff safety injection behavior evaluation form" ,which has been verified to have good reliability and validity,and can provide a basis for scientifically evaluating the safe injecting behavior of nursing staff.2.The investigation results of the safety injection behavior of nursing staff in a Three-A hospital and a Two-A hospital in Hefei,Anhui Province,showed that "hand hygiene" , "sharps injuries prevention" and "checking" were weak links in the process of injections by nursing staff.Therefore,it is suggested that managers should pay more attention to them and intensify their efforts to improve the safety injections of nurses,so as to improve the quality of clinical safety injection.
