Study On Empowerment Level And Its Influencing Factors Of Inpatients With Coronary Heart Disease

ObjectivesFor the purpose of maximizing the utilization of medical resources,reducing the medical burden of patient and improving the quality of life,this paper investigated the present situation of empowerment level of patients with coronary heart disease(CHD)and analyzed its influencing factors.This provides theoretical basis for medical staff to carry out secondary prevention of CHD and formulate individualized nursing intervention measures.MethodsFrom November 2017 to June 2018,230 inpatients with CHD in cardiovascular hospital of Henan Provincial People's Hospital were selected by cross-section survey.The data of patients with CHD were collected by patient General data questionnaire,the Chinese Version of the Patient Perceived Empowerment Scale(CV-PPES),General Self-efficacy Scale(GSES),Chinese Version of Chronic Illness Resource Survey(CV-CIRS).Epidata3.1 was used to input data,SPSS17.0 was used for descriptive statistics,univariate analysis,regression analysis.Results1.Current empowerment level of inpatients with CHDThe total score of empowerment in patients with CHD was(44.00 ±4.19),and the total item score was(4.00 ±0.38)points,which was at a medium level.The scores of information items,decision items,individual items and self-management items were(3.6 ±0.60),(4.12 ±0.60),(3.92 ±0.65)and(3.95 ±0.44),respectively.The lowest score is information dimension and the highest score is decision dimension.2.Current general self-efficacy of inpatients with CHDThe scores of general self-efficacy inpatients with CHD were(29.28 ±4.70),and the items were(2.93 ±2.35),which were at a medium level..3.Current status of chronic disease resource support for inpatients with CHDThe total score of chronic disease resource support for inpatients with CHD was(60.74 ±8.78),and the score rate was 57.8%.The support for chronic disease resources in patients with CHD was less than ideal.The score of medical staff support dimension was the highest(3.79 ±0.68),and that of community support dimension was the lowest(2.00 ±1.23).4.Univariate analysis of influencing factors of empowerment level for inpatients with CHDThe CHD inpatients' gender,age,education level,type of medical insurance,whether they or their relatives have medical workers,whether they have received health education,whether they have complied with health education,and whether they are willing to participate in the formulation of health education,the current disease course,all these aspects are empowering the difference and it was statistically significant(P < 0.05).Among them,educational level,marriage,type of health insurance,whether they or their relatives have medical workers,whether or not they have received health education,the current disease course has statistical significance difference in the information dimension score(P < 0.05);gender,age,current disease course have statistical significant difference in the scores of decision-making dimension(P < 0.05),gender has statistical significance difference in the individual dimension(P < 0.05),and the type of care,current disease course have statistical significant difference in the scores of self-management dimension in the course of disease(P < 0.05).5.Correlation between general self-efficacy and empowerment level for inpatients with CHDThere was a positive correlation between self-efficacy and the total score of empowerment(r = 0.399,P < 0.05).At the same time,self-efficacy was positively correlated with each dimension of information,decision-making,individualization and self-management(r = 0.216,0.395,0.268,0.172,P < 0.05).6.Analysis of correlation between empowerment level of CHD inpatients and social resource support of chronic diseasesThe total score of social resource support for chronic diseases was positively correlated with the total score of perceived empowerment scale(PPES)(r = 0.548,P < 0.05).In addition,the health care support dimension,family and friend support dimensions,media and policy support dimensions were positively correlated with the total perceived cognitive empowerment(r = 0.559,0.417,0.402,P < 0.05),and the correlation was higher.The media and policy support dimensions were positively correlated with information(r = 0.456,P < 0.05)and were closely related.The health care support dimension,family and friend support dimensions were positively correlated with decision-making(r = 0.526,0.511,P < 0.05),and the correlation was higher.The medical staff support dimension was positively correlated with individualization(r = 0.476,P < 0.05),and the correlation was close.Neighbor support dimension was positively correlated with self-management(r = 0.553).7.Multiple linear regression analysis of influencing factors on empowerment level of inpatients with CHDMultiple linear regression analysis was performed on the statistically significant variables in the univariate analysis and correlation analysis results.The results showed that: age,current disease duration,education level,media and policy support,medical staff support,family and friend support were the main influencing factor for inpatients with CHD,R2 = 0.567,adjusted for R2=0.529,which explained 52.9% of the total variation of empoerment level in patients with CHD.Conclusions1.The level of empowerment of patients with CHD is at a medium level,and the lowest score among the scores of each dimension is the information dimension.2.The self-efficacy of the inpatients with CHD is at a high level,and self-efficacy is positively correlated with the level of patient empowerment.3.The chronic disease resource support is low in inpatients with CHD,and Chronic disease resource support is positively correlated with patient empowerment.4.Multiple linear regression analysis showed that age,education level,disease course,medical staff support,family and friend support,media and policy support were the main factors influencing the empowerment level of the inpatients with CHD.
