Investigation Of Effective Utilization Rate Of Erupted Wisdom Teeth

Objective: The whole process of the crown from moving the gum to the occlusal contact is called eruption,and wisdom teeth have a variety of outcomes during their entire development.These include impaction,partial eruption and complete eruption.Most of the partially erupted wisdom teeth may cause dental caries in their neighbors,pericoronitis of wisdom teeth,and so on,and should be removed clinically.Even if some wisdom teeth can completely erupt,it is very difficult for them to establish a perfect occlusal relationship with the teeth in opposite jaw.No occlusal contact(locked jaw),point occlusal contact and incomplete occlusal contact may occur and result in decay because of insufficient self-cleaning.Even if occlusal relationship can be established,self truma such as buccal bite,tongue bite,etc.may arise due to the deep overjet of the wisdom teeth.This means that the function and utilization erupted wisdom teeth remains as a question.However,in clinical practice,it is still controversial whether to carry out prophylatic extraction of wisdom teeth.In this study,the effective utilization rate of erupted wisdom teeth was evaluated.Methods: Oral examination was performed on subjects aged from 20 to 50 years old in physical examination center of the Yijishan Hospital to investigate(1)the eruption states of wisdom teeth(non-eruption,partial eruption,complete eruption),(2)the occlusal states(non-contact,point contact,incomplete contact),(3)pulposis of wisdom teeth or not,(4)pathogenic factor of the second molar or not and(5)self-truma factor(eg.buccal bite)or not.With panoramic film and dental model,the above-mentioned indexes were collected to analyze the effective utilization rate of the erupted wisdom teeth to provide the basis for the prophylactic extraction of wisdom tooth.Results:1.Among the 502 patients in this study,1509 wisdom teeth were erupted,91 of which effectively functioned without problems,and the effective utilization rate of erupted wisdom teeth was 6.03.2.Among the 502 patients in this study,263 of whom were male and 239 female.The erupted wisdom teeth in male subjects was 705,56 of which functioned effectively,and the effective utilization rate was 7.04%(56/705),the erupted wisdom teeth in female subjects was 714,35 of which functioned effectively,and the effective utilization rate was 6.03%(35/714)female.The effective utilization rate was different between male and female subjects(P>0.05).3.Among the 752 maxillary erupted wisdom teeth of the 502 patients in this study,44 maxillary erupted wisdom teeth functioned properly and the effective utilization rate was 5.85(44/752),47 of 757 mandibular erupted wisdom teeth functioned properly and the effective utilization rate of mandibular erupted wisdom teeth was 6.21(47/757).The effective utilization rate between upper and lower mandibular erupted wisdom teeth was not statistically different(P > 0.05).4.Among the 91 wisdom teeth with effective utilization in our data,only 52(26 pairs)could function properly under good occlusal relationship,the other 39 did not have healthy corresponding teeth in the opposite jaw.Conclusion1.The average effective utilization rate of erupted wisdom teeth was 6.44%.2.There was no difference in the effective utilization rate of erupted wisdom teeth between male and female.3.There was no difference in the effective utilization rate of eruption wisdom teeth between the upper and lower jaw.4.Most of the erupted wisdom teeth should prophylacticly removed because of low utilization rate.
