Clinical Observation And Immunological Mechanism Analysis Of Huangdi Decoction In Treating Baby Eczema

Objective:To evaluate the clinical efficacy of Huangdi Decoction in the treatment of infantile eczema,and to analyze the possible mechanism of Huangdi Decoction in treating infantile eczema from the perspective of lymphocytes and immunoglobulin.Method:112 infants with eczema in outpatient clinics were randomly divided into treatment group(Huang Di Tang treatment)and control group(hydrocortisone butyrate intervention),56 patients in each group.Two groups were evaluated after two weeks of treatment on clinical efficacy,safety,and recurrence rate at 1 month and6 months after treatment;Flow Cytometry was used to measure CD4+ cells,CD8+cells,and B cells in peripheral blood of children with eczema before and after treatment,the levels of immunoglobulins lgG,lgA,lgM and lgE in peripheral blood before and after treatment were measured by immunity turbidimetry.Result:(1)The effective comparison between the two groups: the therapeutic effect of the control group was equivalent,94.64% and 91.07,respectively,the difference was not statistically significant,p>0.05;(2)Adverse events occurred: During the treatment,1 patient with skin redness after treatment occurred in the treatment group;3 cases of adverse reactions occurred in the control group,1 patient developed skin redness and swelling,and 2 patients showed smooth and smooth skin at the medication site.After statistical analysis,there was no significant difference in the incidence of adverse reactions between the two groups,P>0.05.(3)The recurrence rate of the two groups: the recurrence rate at the 1 month after the end of treatment in the treatment group and the control group were 17.86%and 39.29%,respectively,and the difference was statistically significant.P<0.05 at 6months,the recurrence rates of the two groups were 32.14% and 55.35%,respectively,and the difference was also statistically significant,P<0.05;(4)Distribution of main lymphocytes: After receiving Huangdi Decoction,the percentage of CD4+ cells(Th)in peripheral blood of children was significantly higher than that before treatment,P<0.05,the difference was statistically significant;peripheral blood CD4+ cells after intervention(the proportion of Ts)was significantly lower than that before treatment,P<0.05,and the difference was statistically significant.The proportion of peripheral blood hematoma CD19+ cells did not change significantly before and after treatment,P>0.05;(5)Immunoglobulin expression level: IgE levels in peripheral blood of children with eczema were significantly lower than those before treatment,P<0.05,the difference was statistically significant;and immunoglobulin lgG in peripheral blood There was no significant change in the levels of lgA and lgM before and after treatment,P>0.05.Conclusion:The treatment of infantile eczema with Huangdi Decoction is effective,and it has the same effect as hydrocortisone butyrate.It is safer and better than hydrocortisone butyrate in recurrence rate.It is worthy of clinical push.Huangdi may pass Regulate CD4+/CD8+ balance and/or down-regulate the expression of IgE to participate in the immunological regulation of children with eczema for therapeutic purposes.
