Backgrounds and ObjectiveWith the development of social economy,the residents' life style has changed profoundly.In the context of aging society and accelerating urbanization,cardiovascular disease has become the main cause of disability and premature death in China and even the whole world.The 2017 China cardiovascular disease report pointed out that the prevalence of cardiovascular disease in China is in a continuous rising stage.According to previous surveys,the current number of cardiovascular disease patients in China is 290 million,including 1.1 million coronary heart disease,which has become the first cause of death for urban and rural residents.As a age-related syndrome,the incidence of sarcopenia is also increasing.Some foreign studies have shown that sarcopenia is involved in the production of cardiovascular diseases through a series of mechanisms,such as insulin resistance,mitochondrial dysfunction,oxidative stress,adipokines and inflammatory factors.At present,the relationship between the two is not clear,whether they exist independently or affect each other.This study mainly analyzed the correlation between coronary heart disease and sarcopenia in elderly hospitalized patients,further explored the relationship of them,and understood the influence of sarcopenia on coronary heart disease in the elderly,which is a great significance for the comprehensive management of elderly patients with coronary heart disease.Methods1.Study Subjects:119 patients were randomly selected to be suspected diagnosis with coronary heart disease in the cardiology ward of The 305 Hospital of People's Liberation Army from January 2018 to January 2019.2.Study methods:(1)questionnaire survey:questionnaire survey was conducted on all enrolled patients.(2)grouping:all enrolled patients underwent coronary angiography and were divided into coronary heart disease group and non-coronary heart disease group according to the results.Before coronary angiography,all patients were screened for sarcopenma and then divided into sarcopenic group and non-sarcopenic group according to the results.3.Statistical analysis:IBM SPSS20.0 statistical software was used for analysis.The measurement data were expressed in meanąstandard or median,and t test was used to compare measurement data of normal distribution between the two groups.The counting data were expressed as a percentage,and the comparison between groups was conducted by x2 test.Pearson or Spearman correlation analysis was used to test the correlation of relevant data.For multifactor analysis,logistics regression analysis was used,and P<0.05 was considered statistically significant for all statistical methods.Results1.Comparison of general information between sarcopenic group and non-sarcopenic groupGeneral data(gender,age,weight,height,BMI,low-density lipoprotein cholesterol,fasting blood glucose,smoking level,drinking level,exercise frequency,and hypertension)of patients between sarcopenic group and non-sarcopenic group were compared,and the differences between the two groups were not statisticallysignificant(P>0.05),body fat rate,abdominal visceral fat area and basal metabolic rate were compared between the two groups,and the results indicated that there were statistical differences in each index between the two groups(P<0.05).2.Correlation analysis of skeletal muscle mass index with body fat rate,abdominal visceral fat area and basal metabolic rateThe correlation between skeletal muscle mass index and body fat rate,abdominal visceral fat area and basal metabolic rate was analyzed.The results showed that skeletal muscle mass index was negatively correlated with body fat rate(r=-0.361,p=0.000).It was also negatively correlated with abdominal visceral fat area(r=-0.370,p=0.000).It was positively correlated with basal metabolic rate(r=0.331,p=0.000).3.The incidence of coronary heart disease was compared between sarcopenic group and non-sarcopenicThere were 26 cases in sarcopenic group,including 23 cases of coronary heart disease,93 cases in non-sarcopenic group,including 48 cases of coronary heart disease.The incidence of coronary heart disease was 88%in sarcopenic group and 52%in non-sarcopenic group.The incidence of coronary heart disease in the sarcopenic group was significantly higher than that non-sarcopenic group.4.To compare the incidence of coronary heart disease between the two groups with different genderThe incidence of coronary heart disease in male patients was compared between the two groups,and the difference was statistically significant(P = 0.01).The incidence of coronary heart disease in female patients was compared between the two groups,and the difference was statistically significant(P=0.02).5.The correlation between skeletal muscle mass index and the degree of coronary artery disease was compared in patients both have sarcopenia and coronary heart disease,and no significant correlation was found between them(P?0.08).6.Binary logistics regression analysis was performed for the correlation indexes of sarcopenia and coronary heart disease,and the results showed that skeletal muscle mass index was a risk factor of coronary heart disease(P=0.001),which indirectly suggested that sarcopenia was a risk factor of coronary heart disease.7.Comparison of the incidence of coronary heart disease between the two subgroupsAccording to the visceral fat area of the abdomen,sarcopenic group and non-sarcopenic group was divided into different subgroups.The incidence of coronary heart disease among different subgroups was compared,and the results showed that there were statistical differences between the two subgroups(P=0.02).Conclusions1.There were statistically significant differences in body fat rate,abdominal visceral fat area,and basal metabolic rate between the two groups,and the skeletal muscle mass index showed a certain correlation with the above three indexs.2.Sarcopenia is a risk factor for coronary heart disease,and patients with sarcopenia are more likely to have coronary heart disease than patients without sarcopenia.3.The severity of muscle mass loss in patients with sarcopenia is not correlated with the severnty of coronary artery disease in patients with coronary heart disease.4.Sarcopenia combined with abdominal visceral obesity is more likely to cause coronary heart disease than simple abdominal visceral obesity.
