Objective To explore the structure and distribution characteristics of cognitive function in epileptic children in the Chinese edition of Wechsler Intelligence Scale for Children-The Fourth Vision(WISC-IV),and to analyze the specific performance and weaknesses of cognitive function in epileptic children.Methods This method belongs to retrospective study.Children with epilepsy diagnosed by the First Affiliated Hospital of China University of Science and Technology and the Hefei First People's Hospital epilepsy specialist outpatient clinic and children's health clinic were selected.According to the International League Against Epilepsy(ILAE)criteria,we excluded the history of severe jaundice,intracranial hemorrhage,hypoxic ischemic disease,cortical dysplasia and history of bilirubin encephalopathy;postnatal cerebral palsy,developmental delay,autism spectrum disorder and other neurological diseases.71 cases were included in the epilepsy group.In addition,in the same time period,normal healthy children matched with the age and sex of the epilepsy group were selected as a control group according to the random sampling method,a total of 60 cases.By using WISC-IV test to evaluate the structure and distribution characteristics of cognitive function of all observed children,the weaknesses were identified and further compared.At the same time,the basic and clinical information of children in epilepsy group was collected,mainly including the choice of educational form,family history,initial onset age,duration of seizures,type of seizures,nearly one year frequency of seizures,duration of seizures and antiepileptic drugs(AEDs)treatment(drug selection,medication time,adverse reactions),etc.,to explore the impact of epilepsy on cognitive function especially weak functional characteristics of children with vulnerable factors.Results According to the WISC-IV test showed that the level of total intelligence quotient(FSIQ)in epilepsy group was mostly concentrated in the critical state(to the left of the transverse axis),while the level of total intelligence quotient(FSIQ)in control group was mainly concentrated in the middle level(to the middle of the transverse axis).From the analysis of the specific results,we were found that children in epilepsy group on the scores of in the total intelligence quotient(FSIQ)and four index,such as Verbal comprehension index(VCI),Perceptual reasoning index(PRI),Working memory index(WMI),Processing speed index(PSI),General Ability Index(GAI),Cognitive Proficiency Index(CPI)and on the 10 core subtest were significantly lower than those in the control group,(P < 0.05),and the scores of cognitive proficiency index(CPI)were lower,especially the processing speed index(PSI).In addition,when studying the relevant influencing factors affecting cognitive proficiency dysfunction in children with epilepsy,the severity of the processing speed index(PSI)was taken as the dependent variable of the study.The results showed that the age of onset of epilepsy,mother's educational level,duration of seizure,type of seizure,frequency of seizures in recent one year,course of disease,treatment of AEDS,medication time and education form could affect the cognitive proficiency of children with epilepsy,including the age at the onset of epilepsy,the different forms of education,the level of mother's education and the type of seizures,may be the risk factors of aggravating the severity of cognitive proficiency in children with epilepsy.Conclusion The application of WISC-IV to assess the cognitive function of children with epilepsy can visually identify the strengths and weaknesses of children,which is not only beneficial to the diagnosis and selection of targeted treatment programs for clinical specialists,but also to help families with epilepsy Children choose special education and implement early cognitive intervention to provide objective basis.
