Purpose Implant denture has been widely used because of its good chewing ability and no damage to adjacent teeth.However,due to the absorption of alveolar bone and the displacement of adjacent teeth in edentulous area,the conventional diameter implants are limited.The use of narrow diameter implants can solve the problem of insufficient space between edentulous teeth and reduce bone reconstruction.However,the mechanical properties of narrow-diameter titanium implants commonly used in clinic are limited,and fatigue fracture may occur under long-term stress.Therefore,narrow-diameter and high mechanical strength implants may be one of the best choices for insufficient three-dimensional space.Roxolid titanium-zirconium alloy implant with3.3mm diameter is the latest implant introduced by Straumann Company.It is reported that its mechanical strength reaches 4.1mm pure titanium implant strength.In this paper,the clinical effect of titanium-zirconium narrow-diameter implants for the restoration of bone deficiency sites in anterior teeth aesthetic area was evaluated retrospectively.Methods From June 2015 to October 2016,30 patients(13 males and 17 females,aged25-66 years)with lip-lingual alveolar ridge bone mass less than 5.5mm were selected from the implant center of Anhui Stomatological Hospital for dental implant restoration due to missing teeth in the aesthetic area of anterior teeth.A total of 38 Roxolid Ti-Zr implants(Straumann,Switzerland)with a diameter of 3.3 mm were implanted.21 implants with a length of 10 mm and 17 implants with a length of 12 mm were implanted.Clinical and CBCT examinations were performed 12 months and 24 months after implant placement.The success rate of titanium-zirconium 3.3mm narrow-diameter implants,edge bone resorption,soft tissue and aesthetic effects,andprosthesis complications were evaluated.Soft tissue and aesthetic effects were compared by modified plaque index(m PLI),modified gingival sulcus bleeding index(m SBI),Peri-Implant Probing Depth(PPD)and pink aesthetic index(PES).To evaluate the2-year clinical effect of titanium-zirconium narrow-diameter implants in the aesthetic area of anterior teeth.Results The 2-year success rate of the implant was 100%.At 12 months after implant placement,the average marginal bone resorption height was 0.51±0.20 mm,and the24-month was 0.59±0.18 mm.The edge bone height did not change significantly(t=1.381,P=0.178).The PLI of 12 months was 0.73±0.58 and that of 24 months was0.70±0.65(t=-0.680,P=0.499).There was no significant difference.Oral hygiene was good during the observation period.The value of m BI in 12 months was 0.23±0.50 and that in 24 months was 0.20±0.48(t=0.513,P=0.610).The difference was not statistically significant.The gingival health was good.The 12-month PD value was 2.03±0.76 mm and 24-month PD value was 2.00±0.79,(t=0.339,P=0.736).There was no significant difference.The PES value was 9.77±1.48 in 12 months and 10.77±1.50 in 24 months(t=0.426,p=0.672).Compared with 12 months,PES increased in 24 months,but there was no significant difference.The overall aesthetic effect was good and no complications occurred after repair.Conclusion In this two-year clinical retrospective study,Ti-Zr 3.3mm narrow-diameter implant is suitable for patients with insufficient bone mass in the aesthetic area of anterior teeth,with a high success rate and satisfactory aesthetic effect.It is a safe and effective choice for implant prosthesis.As the follow-up time of this experiment is 2years,the clinical efficacy of 3.3 mm narrow-diameter titanium-zirconium implants in the medium,long-term and large sample size remains to be further observed.
