Objective To investigate the relationship between complement factor H(CFH)and Complement factor H-associated protein(CFHRP)1,2,3,and 5 in plasma and urine and IgA nephropathy.o study the value of complement factor H and complement factor H related proteins 1 and 5 in plasma in the auxiliary diagnosis of IgA nephropathy.Methods 153 patients diagnosed as IgA nephropathy by renal biopsy in Renal Hospital of Xinjiang Uygur Autonomous Region and 150 healthy volunteers were enrolled.The Concentrations of CFH Factor and CFHRP1,2,3 and 5 in plasma and Urine were measured by Enzyme-Linked Immunosorbent Assay(ELISA).And patients' baseline,clinical and pathological data were recorded at the same time.For the 217 enrolled cases(IgA nephropathy group,other kidney disease group,and healthy control group),the concentrations of complement factor H and complement factor H related protein 1and 5 in plasma were determined by enzyme-linked immunosorbent assay(ELISA).SPSS20.0 software was used to analyze the data and the receiver's working characteristic curve was drawn to judge the diagnostic value of the above three indicators.Results Plasma CFH levels are inversely proportional to serum creatinine,urea nitrogen,and urinary protein,but are proportional to serum albumin.The higher the Oxedo grade MEST score and the stage of chronic kidney disease,the lower the level of complement factor H in plasma of patients with IgA nephropathy,and the higher the concentration of complement factor H in the urine and the level of complement factor H-related protein 5 in plasma.The concentration of complement factor H in the IgA nephropathygroup was lower than that in other kidney disease groups and healthy controls,but the concentrations of complement factor H-related proteins 1 and 5 were significantly higher than those in the other two groups,and the difference was statistically significant(P <0.05).The diagnostic sensitivities of complement factor H and complement factor H-related proteins 1,5 were 85%,69%,and 60%,respectively,Similarly,the specificities were 71%,77%,and 71%.The receiver operating characteristic curve showed that the area under the curve of all three indicators was greater than 0.7,and the area of complement factor H was the largest(0.862).Conclusion The concentration of CFH and CFHRP1,5 in plasma and urine has a certain correlation with the clinicopathology of IgA nephropathy.Complement factor H and complement factor H-related proteins 1 and 5 in plasma have a certain significance for the diagnosis of IgA nephropathy,of which complement factor H has the highest diagnostic value.
