| The "YangyinHewei Formula" prescription has the medical effects of nourishing yin,reliving stomach stress and tonifying the spleen.It was also found that the prescription has satisfying treatment on diabetic gastroparesis(DGP).Current project focuses on investigating the theoretical basis and mechanism of the treatment basing on previous experiments and clinical results.Part 1:Theoretical studyWe first reviewed and analyzed the cause,diagnosis and treatment and DGP from the aspect of modern medicine.Although some progress was made in this direction,people still have no specific method to recover or reverse the muscle motions in gastrointestinal tract.On the other hand,studies from the point of view of traditional Chinese medicine have demonstrated their advantages.In Chinese medicine pathogenesis,DGP results from the lack of "yin" in stomach and the weakness in spleen and has the symptoms of "qi" stagnation,indigestion and resistance to moisture.Clinically DGP has been treated by "YangyinHewei Formula"prescription.In this part we theoretically analyzed the prescription from traditional Chinese medicine formulation,and further expounded the mechanism of "YangyinHewei Formula"treatment on DGP.Part 2:Experimental researchObjective:By observing the effect of "YangyinHewei Formula" on HO-1 expression level,macrophage equilibrium and Cajal interstitial cell in the gastrointestinal tract cells of rats,we discussed on how "YangyinHewei Formula" affected DGP,as well as the possible mechanism how the prescription recovers and even reverses the muscle motions.Methods:Intraperitoneal injection STZ method was set up on rat model.The DGP rats were randomly divided into model control,low-dose,medium-dose,high-dose Chinese medicine control,and western medicine mosapride control.A normal rat group was also set up.There are 12 rats in every group.The DPG rats were gavaged with D1 water(model control),lose-dose Chinese medicine(7.8g/kg/d),medium-dose medicine(15.6kg/d),high-dose medicine(31.2g/kg/d)as well as western medicine(2.5mg/kg/d).The normal rat group was gavaged with same volume of DI water of 10ml/kg.During the 6 weeks,the conditions of the rats were checked daily.After the last gavage,the blood was taken for measurement of different indicators.The rats were put to death and all stomach tissues were taken.Results:?Lower gastric emptying rate was observed in the model controls,the controls with Chinese medicine and western medicine all have significant improvement in gastric emptying rate.?Chinese medicine and western medicine controls all have recovered ICC disorder compared with model group.?Model control has higher macrophage M1/M2 ratio than normal group.Significant difference(P<0.05)was observed between the M1/M2 ratio in western medicine and Chinese medicine of different doses.High-dose.YangyinHewei Formula" decreased Ml/M2 ratio more than western medicines.?The Chinese medicine control groups improved HO-1 content compared with western medicine control,and they have significant difference compared with model control group(P<0.05).From the value results,the level of HO-1 improvement increases with the increasing dose of "YangyinHewei Formula".Some amount dependence can be observed.Conclusion:Yangyinhewei Formula can relieve the disorder of ICC in gastrointestinal tract cells of rats.It also improves gastric emptying rates,the ratio of macrophage typing and HO-1 expression level.Especially,the effect on macrophage typing was better that control with western medicine.Apart from the understanding basing on traditional Chinese medicine,"Yangyinhewei Formula" worked on DGP by controlling macrophage typing into Ml/M2,which further promotes HO-1 expression and improves stomach motion.The functioning mechanism is very possible to come from the control over macrophage M1/M2 ratio.Our study revealed part of the mechanism for "Yangyinhewei Formula" prescription to work on DGP.This provides scientific clues that Chinese medicine has advantages in treatment by exerting real physiological effect. |
