Objectives:To investigate the status and influencing factors of attitude to ageing and frailty of the elderly in pension service institutions,and to explore the correlation between them,so as to provide theoretical basis for improving the attitude to ageing and frailty of the elderly in pension service institutions and promoting the development of healthy ageing and active ageing.Methods:Convenient sampling method was adopted to conduct a survey on the elderly aged 65 and above in four pension institutions in changchun from march to June,2018.The survey tools were attitude to ageing questionnaire,frailty phenotype,Chinese version of simple mental state scale and general information questionnaire.Epidata3.1 was used for data entry,and SPSS21.0 was used for data statistics and analysis.Frequency,percentage,mean and standard deviation were used to describe the general data of the elderly in nursing institutions.Mean,standard deviation,frequency and percentage were used to describe the attitude to ageing scores and frailty of the elderly in nursing institutions.Univariate analysis,multivariate linear regression and logistic regression were used to analyze the influencing factors of attitude to ageing and frailty of the elderly.Appropriate test methods(anova,X2 test,Nonparametric rank sum test)were selected according to data type,data distribution and homogeneity of variance.Spearman correlation analysis was used to explore the correlation between attitude to ageing and frailty of the elderly in pension service institutions.Results:1.The average score of the attitude to ageing of the elderly in pension institutions was(79.10±21.33),and the score of each dimension was: the average score of psychosocial loss dimension was(27.28±8.66),the average score of physicalchange dimension was(27.32±8.16),and the average score of psychological gain dimension was(24.50±8.59).2.Univariate analysis showed that under the conditions of drinking,exercise,sleep problems,vision problems,types of chronic diseases,types of drugs,and self-rated physical health subgroups,the difference in attitude to ageing score of the elderly in pension institutions was statistically significant(P<0.05).The results of multivariate regression analysis showed that exercise status and self-rated physical health level were the main influencing factors of the attitude to ageing of the elderly in pension institutions.3.There were 82 cases of physical frailty among the elderly in pension institutions,accounting for 42.9%,and 109 cases of non-frailty,accounting for 57.1%.The average score of FP was(2.10±1.31),and the proportion of FP indicators was from high to low: decreased grip strength,decreased walking speed,decreased physical activity,fatigue,and unexplained weight loss.4.Single factor analysis showed that movement situation,sleep problems,vision problems,variety of kinds of chronic disease,medication,and self-reported health group,the pension institution of incidence of the elderly physical frailty differences statistically significant(P < 0.05),logistic regression analysis,according to the results of the influence factors of the elderly's physical frailty pension institutions mainly including the movement of the elderly and self-reported health level.5.In this study,there were 47 cases of elderly people with cognitive frailty,accounting for 24.6%,and 144 cases without cognitive frailty,accounting for 75.4%.Single factor analysis results show that the cultural degree,medical treatment charge payment,movement,sleep problems,self-reported health group,the pension institution of incidence of the elderly cognitive frailty differences statistically significant(P < 0.05),logistic regression analysis results show that the influence factors of cognitive frailty in the elderly pension institutions mainly including the old people's cultural level and self-reported physical health level.6.The results showed that pension institutions attitude to ageing and negatively correlated with physical frailty elderly(r =-0.630),in addition to the loss ofpsychosocial dimensions and were not associated with unexplained weight loss,attitude to ageing each dimension with physical frailty and each index showed a negative correlation(P < 0.05),pension institutions old attitude to ageing and negatively correlated with cognitive frailty(r =-0.474),all the dimensions and negatively correlated with cognitive frailty(P < 0.05).Conclusion:1.The ageing attitude of the elderly in pension service institutions is at the medium level.The ageing attitude scores of the elderly with different drinking,exercise,sleep problems,vision problems,types of chronic diseases,types of drugs and self-assessment of physical health are different.2.The frailty of the elderly in nursing institutions is more serious,and the incidence of physical and cognitive frailty is much higher than that of the elderly in the community.The main influencing factors of physical frailty are exercise and self-assessment of physical health;The main influencing factors of cognitive frailty are education level and self-assessment of physical health.Relevant staff in pension service institutions should focus on the frailty of the elderly,and provide relevant intervention measures to reduce the incidence of frailty and improve the healthy life span of the elderly.3.The ageing attitude of the elderly in nursing institutions is negatively correlated with physical frailty and negatively correlated with cognitive frailty,indicating that the more positive the ageing attitude of the elderly,the lower the incidence of ageing in the elderly.Therefore,the staff of pension service institutions should take corresponding measures to enhance the psychological construction of the elderly,eliminate negative ageing emotions,and then improve the frailty,promote healthy ageing and active ageing.
