Background:Laparoscopic right hemicolectomy has been standard procedure for the resection of the colon,but recently there are many different approaches,in our study we showed only two challenging approaches of right laparoscopic hemicolectomy(Medial to Lateral vs Lateral to medial approach).These two procedures gained popularity and is now common procedures performed for the treatment of colon cancer.These two methods are more commonly used in laparoscopic right colon surgery.The advantages and disadvantages of the lateral approach and the medial approach are controversial,Different techniques can be chosen for different patients and doctors.Objective:To evaluate the advantages of medial approach and lateral approach in laparoscopic right colectomy for colon cancer from the aspects of complication rate,hospital stay,operative time,lymphatic resection,and surgical complications.The second objective was to compare the intraoperative and postoperative variables in the two groups to assess whether the endogenous approach was superior to the lateral approach.Research methods:From January 2015 to December 2018,the clinical data and records of 450 patients with right colon cancer admitted to the China-Japan Union Hospital of Jilin University were retrospectively reviewed and analyzed.According to the method,they were divided into two groups: the first group was the L-M and the second group was the M-L.Patient records will be queried according to different groups to obtain the results required for the study.Before the study began,it was approved by the Ethics Committee of China Japan union Hospital of Jilin University.The analytical data was analyzed using SPSS.22 software.Results Result :There was no difference in mean blood loss between patients who underwent LMA and OR(178mL vs 158 mL,respectively,P=0.918).The mean operating time for MLA was significantly longer than that for LMA(165 minutes vs 115 minutes,respectively P<0.001).but we believe laparoscopy of right hemicolectomy have theirown technical challenges and need to learn the curves the more you practice the more you make the operation time short.or number of positive lymph nodes(MLA,1.0vs.LMA,0.7,P =.12)Further subgroup analysis for stage II and stage III colon cancer revealed that for stage II cancer,there was no difference in number of harvested lymph nodes(MLA,17.oncological results after the surgical for stage II and III colon in our study We found no significant differences in terms of the number of retrieved lymph nodes.Also there were no significant differences in the median number of retrieved lymph nodes(LMLA,17.0vs.LLMA,16.9,P =.86)or number of positive lymph nodes(LMLA,1.0vs.LLMA,0.7,P =.12)between the two groups,as shown in Table 3.Further subgroup analysis for stage II and stage III colon cancer revealed that for stage II cancer,there was no difference in number of harvested lymph nodes(LMLA,17.0vs.LA,17.6;P =.55)between the two groups.
