Construction Of Nursing Safety Evaluation Indicator System Of Artifical Airway For Patients With Severe Neurological Diseases

Objective:The purpose of this study is to finish the construction of nursing safety evaluation indicator system of artificial airway for severe neurological patients,,to improve the quality of artificial airway nursing and provide the reference for clinical management.Method:(1)The framework of safety evaluation index system for artificial airway nursing in neurointensive care unit is preliminarily constructed by literature review and qualitative interview.(2)After two rounds of Delphi expert consultation,the evaluation index system is established,and the weight of the index is determined by analytic hierarchy process.(3)According to the evaluation index system,a questionnaire on the safety status of artificial airway nursing for severe neurological patients was compiled,and the reliability and validity of the questionnaire were tested to realize the confirmatory investigation of the index system.Result:(1)After two rounds of expert inquiry,the questionnaire recovery rate is 100%,the authoritative degree coefficients of experts are 0.93 and 0.95 respectively;first-level index coordination coefficients were 0.18 and 0.43,second-level index were 0.34 and 0.48,.the third-level index are 0.34 and 0.48,the coordination coefficients of three-level index importance are 0.24 and 0.40,formula rationality are 0.19 and 0.28,index collection operability are 0.23 and 0.34,and the total coordination coefficients of all levels index are0.22 and 0.34.The nursing safety evaluation index system of artificial airway includes three first-level indicators,ten second-level indicators and 34 third-level indicators.(2)On the basis of index importance score,the weight of each index is calculated and the consistency test is carried out.By analytic hierarchy process,the weight coefficients of the structural index,process index and final index are 0.1365,0.6250 and 0.2970.The CR values at all levels of indicators are less than 0.1,which shows that the consistency test results of all levels of indicators are satisfactory.(3)The test survey results of the index system:the Cronbach's?coefficient of the first index is between 0.926 and 0.980;the split-half reliability is between 0.864 and 0.980;the Cronbach's?coefficient of the second index is between 0.802 and 0.964,the split-half reliability is between 0.790 and 0.969;the Cronbach's?coefficient of the third index is0.944 and the split-half reliability is 0.762;which shows that the reliability of the index system is high.The content validity index is 0.97.Exploratory factor analysis obtains three factors,KMO=0.951,c~2=7433.524,P=0.000.The loading range of each factor is 0.712-0.958,indicating that the reliability and validity of the index are good.Conclusion:In this study,nursing safety evaluation indicator system of artificial airway for patients with severe neurological diseases is constructed,which included three first-level indicators,ten second-level indicators and 34 third-level indicators.The weights of indicators at all levels were determined,and the index system was validated.
