Objectiveto investigate the correlation between serum Lp-PLA2 level and cognitive impairment in patients with ischemic leugoencephalopathy and analyze its risk factors.MethodsSixty-nine patients with ischemic leucoencephalopathy diagnosed by head MRI in the department of neurology,china-japan friendship hospital of jilin university from January 2017 to December 2018 were included.The patients were assessed by the Montreal Cognitive Assessment MoCA scale(Vascular disease assessmentscale-cognitive subscale)VaDAS-cog)clear of 37 patients with cognitive impairment(mild cognitive impairment,15 cases of severe cognitive impairment 22 cases),32 cases of patients with cognitive impairment with age-related white Matter Changes(Age-RelatedWhite Matter Changes ARWMC)scale for ischemic cerebral white Matter lesion severity score collection with and without cognitive impairment,two groups of patients' demographic data and clinical data to detect serum Lp-PLA2 folic acid,vitamin B12,homocysteine and low density lipoprotein cholesterol,total cholesterol levels by SPSS23.0 statistical analysis,measurement and counting data were analyzed by t test and chi-square test,respectively.ResultsARWMC scale,ischemic cerebral white matter lesions are divided into the relation between(6 points)and mild(< 6 minutes),use MoCA scale to assess patients' cognitive function,compared two groups of cognitive level,the results showed that cognitive dysfunction in patients with ischemic cerebral white matter lesions moderately severe relatively mild group,two groups are significant difference(p =0.00),the severity of cerebral white matter lesions and negatively correlated with cognitive level,r = 0.954;The ARWMC scale,ischemic cerebral white matter lesions are divided into the relation between(6 points)and mild(< 6 points),to compare two groups of indicators,the results showed that compared with mild group,the relationbetween group of older,Lp-PLA2 level higher,lower vitamin B12 levels,the difference was statistically significant(p < 0.05),while gender smoking drinking hypertension diabetes folic acid homocysteine low density lipoprotein cholesterol,total cholesterol is no difference(p > 0.05);Use MoCA rating scale,ischemic cerebral white matter lesions are divided into cognitive impairment in patients with group(<26 points)and the cognitive impairment group(26 points),compared two groups of indicators,the result shows: the older,cognitive impairment group Lp-PLA2 level and higher incidence of diabetes,folic acid and vitamin B12 levels are lower,the difference was statistically significant(p < 0.05),cognitive levels in patients with ischemic cerebral white matter lesions with Lp-is inversely related to the level of PLA2 and r = 0.965.ConclusionHigh lp-pla2 level and vitamin B12 deficiency are independent risk factors for ischemic leukoencephalopathy and ischemic leukoencephalopathy with cogn itive impairment.There was a significant negative correlation between cognitive level and lp-pla2 level in patients with ischemic leukoencephalopathy.
