Research On The Risk Factors Of Cognitive Impairment In Patients With Acute Ischemic Stroke

Objective:To investigate the major risk factors of cognitive impairment in patients with acute ischemic stroke.Methods:A total of 778 patients with acute ischemic stroke admitted in Neurology Department of The First Affiliated Hospital of Zhejiang University and the other 6 regional hospitals during November 2013 to August 2016 were recruited in this study.General demographic information,medical history,physical examination,vascular risk factors,imaging data were collected.The cognitive function was assessed by the Mini-Mental State Examination(MMSE),Alzheimer's Disease Assessment Scale-cognitive subscale(ADAS-Cog).Magnetic resonance imaging was used to calculate patients'infarction.Recruited patients were divided into vasular cognitive impairment(VCI)group and no cognitive impairment(NCI)group according to cognitive impairment diagnosis standards.Patients with VCI comprised of those with vascular mild cognitive impairment(VaMCI)and vascular dementia(VaD).Inter-group differences and correlations of factors mentioned above were analyzed.Results:Among 788 cases of patients with acute ischemic stroke,VCI accounted for 62.6%which was composed of 37.3%of VaMCI and 25.3%of VaD.Logistic regression analysis showed that compared to NCI group,old age,low educational status,heart disease history,stroke severity(NIHSS)scores and temporal infarction were independent risk factors for VCI.Further analysis show that,old age,low educational status,heart disease history,basal ganglia infarction were independent risk factors for VaMCI.Old age,low educational status,left infarction,frontal and temporal infarction were independent risk factors for VaD.Conclusion:62.6%patients with acute ischemic stroke have cognitive impairment.Old age,low educational status,stroke severity(NIHSS)scores,location of the infarction were independent risk factors for post stroke cognitive impairments.
