Objective: to analyze and compare the early clinical efficacy of internal fixation and joint replacement in the treatment of femoral intertrochanteric fractures in elderly patients.Methods: 78 elderly patients(76.86 ± 9.27 years old)with femoral intertrochanteric fractures who received surgical treatment in the hand surgery department of the first affiliated hospital of guangxi medical university from November 2012 to August 2018 were selected and divided into the internal fixation group and the joint replacement group according to the inclusion and grouping criteria.Cases of patients data were retrospectively analyzed,a total of 66 patients were lost to follow-up and not have complete clinical data,the internal fixation group of 41 cases,25 cases,joint replacement group collects two groups of operation time,intraoperative blood loss,such confinement,down load time,postoperative 3 months and 6 months after Harris hip score,the situation of the postoperative complications,analyzed internal fixation and joint replacement in the treatment of elderly patients with femoral fractures between rotor early clinical curative effect.Results: the Harris score of the hip joint 3 months after surgery in the internal fixation group was lower than that in the joint replacement group,and the time to the ground was later than that in the joint replacement group.The operation time and hospital stay in the internal fixation group were longer than that in the joint replacement group,and the difference was statistically significant(P<0.05).There was no significant difference between the two groups in hip Harris score 6 months after surgery,excellent and good rate of hip function 6 months after surgery,and intraoperative blood loss(P>0.05).Conclusion: internal fixation and joint replacement are effective methods for the treatment of intertrochanteric fractures in the elderly.Joint replacement is suitable for the elderly or intertrochanteric fracture with hip disease. |
