Objective:Based on the patients of cognitive impairment after AIS screening,analyze the related factors of its occurrence,to provide evidence of VaD etiology research.At the same time,analyze the TCM syndrome,to illuminate the relationship between the cognitive impairment after acute ischemic stroke and TCM Syndrome Differentiation-Type,to provide an evidence for prevention and treatment for VaD.Methods: This study makes a retrospective analysis of the clinical data about patients who were diagnosis of acute ischemic stroke by MRI in the Jiangmen wuyi Hospital of Traditional Chinese Medicine from September 2016 to August 2018.According to the accepted diagnostic criteria,use Simple Mental State Examination(MMSE)to assess the cognitive function of the subjects,ultimately 168 patients with cognitive impairment(CI)and 114 patients with non-cognitive impairment(NCI)were obtained.Analyzing general informations,vascular risk factors,imaging datas and Lab index in two groups of patients,simultaneously to observe syndrome.Input the datas obtained into Excel to establish a database,the collection of datas using statistical software SPSS 21.0 descriptive statistics.Results:1.In this study,282 patients with AIS were selected,168 of them were cognitive impairment patients,which was accounting for 59.57%.2.Comparing the general informations of the two groups,the age of CI group and the proportion of drinker in CI group was higher than NCI group,the difference of which was statistically significant.There was no significant difference between the two groups in gender,education level and smoking.3.Comparing the vascular risk factors of the two groups,the proportion of stroke history and dyslipidemia in CI group was higher than NCI group,there was statistical difference between two groups.In hypertension,diabetes,and the history of coronary heart disease,there was no statistical difference between the two groups.4.The incidence of frontal lobe infarction in CI group was significantly higher than NCI impairment group,with statistical difference.The two groups of patients in terms of infarction size had no statistical difference.5.Comparing the Lab index of the two groups,the FIB,Hcy in CI group were higher than NCI group,with statistical difference.There was no significant difference in LDL-C,PLT and UA value.6.Logistic regression analysis showed that age,drinking,stroke history,dyslipidemia,frontal lobe infarction,elevated levels of FIB and Hcy were risk factors for cognitive impairment after AIS.7.The common syndromes of CI group were kidney essence deficiency syndrome,phlegm obstructing orifices syndrome,blood stasis in collaterals syndrome and qi-blood deficiency syndrome,and the incidence of qi-blood deficiency syndrome and kidney essence deficient syndrome was higher than NCI group,with statistical difference.At the same time,the occurrence of cognitive impairment was the result of a variety of pathological factors,phlegm turbidity and blood stasis were important factors in this disease.Conclusion:1.In this study,the incidence of cognitive impairment after AIS was 59.57%.2.Risk factors for cognitive impairment after AIS include age,drinking,stroke history,dyslipidemia,frontal lobe infarction,increased levels of FIB and Hcy.3.The common TCM syndromes of cognitive impairment after AIS were kidney essence deficiency syndrome,phlegm obstructing orifices syndrome,blood stasis in collaterals syndrome and qi-blood deficiency syndrome.The basis of the disease were qi-blood deficiency and kidney essence deficient.The important pathological factors in this disease were phlegm turbidity and blood stasis.
