The Strategy Optimization Of Primary Diagnosis In Medical Group

Since China launched a new round of medical reform in 2009,although many achievements have been made,the medical problem still exists.Regardless of urban or rural areas,patients like to flock to tertiary hospitals.The pressure on medical services in large hospitals is high,patients cannot enjoy quality medical services,and grassroots communities lack patient sources,resulting in wasted medical resources.In 2012,the state proposed to develop the medical association model and the primary diagnosis mechanism at the grassroots level,aiming at effectively alleviating the medical problem and further promoting medical reform.It has been implemented all over the country,but its effectiveness has been poor.There is a general lack of rigidity in the first-level diagnosis at the grassroots level,insufficient policy tilting scale,loose medical structure,and no role in promoting the role of the first-instance at the grassroots level.This article is based on this background,taking a municipal medical group in Wenzhou,Zhejiang Province as an example,adopt the qualitative and quantitative research methods,on the basis of introducing the research status and shortcomings of primary care at home and abroad,through literature,interviews,questionnaires,statistical analysis,case studies and other methods to investigate the first visit at the grass-roots level in the medical group.The subjects of study included community residents,doctors and medical institutions in medical groups,analyze the impact of these three subjects in the process of first diagnosis at the grass-roots level.At the same time,combining with the current medical environment in Wenzhou(for example,local policies,medical insurance measures,drug policies and other factors),this paper analyzed the difficulties faced by the first visit at the grass-roots level,and tap the advantages of community health care,on this basis,the conclusions are drawn and suggestions are put forward to help the development of the first clinic at the grass-roots level in the medical group.Through investigation and research,the following results are obtained:1.The age,income,education and type of work of the community residents will influence whether they go to the community health service center for medical treatment.Factors such as distance of community health service center,medical insurance policy and medical level will influence the attraction of community health care to community patients.2.At present,the elderly in the community health service center are mainly based on the elderly,followed by people with low income levels.Community health care has not yet reached the recognition of people at all levels,but it has a certain ability to treat senile diseases and chronic diseases.3.The basic medical foundation is weak,community doctors lack platform and incentives to upgrade,medical level is limited,medical hardware conditions are weak,and grassroots lacks medical service features.4.The internal structure of the medical association is loose,The cooperation between medical institutions has yet to be engrossed,and the first-time consultation at the grassroots level has not played a role in fueling the situation.5.The state lacks a regulatory mechanism,and has weak regulation and control.The policies such as the medical security system and essential drugs are not sufficient for the primary medical services.Research conclusions:The development of the first consultation at the grass-roots level within the medical group helps to integrate the medical resources within the medical group,improving the quality of community medical services,providing convenient,effective and economical primary medical and health services for community residents.However,the first consultation at the grass-roots level is hindered by the level of grass-roots medical services and residents' traditional medical concepts,and there are many difficulties in the implementation process.Suggestion:1.Set up a special medical service for the elderly,and focus the community medical service on the elderly.2.Pay attention to the construction of community talent team,increase investment in medical hardware conditions.3.Improve the construction of medical associations,provide high-quality resources for improving the quality of primary medical services,and rationally guide and achieve a win-win situation.4.The government established a regulatory system,expanded the strengths of medical insurance and the basic drug system,and provided a more appropriate medical environment for the development of primary care at the grassroots level.
