Predictive Factors Of Glucocorticosteroid Treatment Failure In Severe Ulcerative Colitis Patients And Risk Factors For Recurrence

OBJECTIVE: Colectomy is often the end point of patients who have failed hormone therapy in the past;nowadays,with the development of immunosuppressive agents and biological agents,controlling symptoms,reducing recurrence and rehospitalization rate,and maximizing the prognosis of UC patients become the key point of UC diagnosis and treatment at this stage.How to accurately predict the risk factors of UC patients,especially severe UC patients,has become a subject of extensive research worldwide.It is of great significance to explore the predictive risk factors of patients with glucocorticosteroid failure in the new context.METHODS: 69 patients with severe UC who were diagnosed and treated in digestive department of our hospital from August 2011 to September 2016 were included.The data of age,sex,course of disease,colonoscopy performance,past history,serological data and pathological data were analyzed by SPSS 22.0.The hormone levels of UC patients were analyzed by multivariate regression analysis.The early predictors of UC resistance and the factors influencing UC recurrence were analyzed by follow-up data analysis and multivariate regression analysis.RESULTS: Among 69 UC patients,33 were male and 36 were female,the ratio of male to female was 0.91:1.The average age of onset was 45.82 years.The failure rate of hormone therapy for severe UC was 29.0%.The recurrence rate of severe UC patients was 65.2%.Among 69 patients with severe UC,E1 accounted for 26.1%,E2 5.8% and E3 68.1%.According to multivariate regression analysis,the hematochezia and lesion range of patients with D3 treated with corticosteroids are independent risk factors for the failure of hormone therapy in severe UC patients.Chronic recurrence is an independent risk factor for severe UC recurrence.The recurrence of severe UC is not related to CRP,Hb,Alb,age,sex and smoking history.
