Clinical Study On The Effect Of Xingnao Tongdu Acupuncture On Cerebral Blood Flow In Patients With Ischemic Stroke(LAA Type)

Objective:To study the effect of Xingnao Tongdu acupuncture on cerebral blood flow in patients with ischemic stroke(LAA type),and to provide clinical support for Xingnao Tongdu acupuncture in the treatment of ischemic stroke.Methods:The inpatients who were admitted to the First Department of Encephalopathy in the Third Hospital of Shanxi University of Traditional Chinese Medicine from November 2017 to December 2018 were enrolled in the study.According to the order of admission,60 patients who met the criteria were randomly divided into treatment group(n=30)and control group(n=30).Under the premise of basic treatment of encephalopathy,the treatment group adopts the Xingnao Tongdu acupuncture,the control group adopts the traditional acupuncture.The treatment time of the two groups was once a day,each time the needle was kept for 30 minutes,3 weeks was a course of treatment,and 1course of treatment was used for comparative study.Transcranial Doppler(TCD)and hemorheology were performed before and after treatment in the two groups,and to evaluate The national institutes of health stroke scale(NIHSS),and observe the clinical efficacy.Results:(1)Comparison of cerebral hemodynamics:After treatment,the mean velocity(Vm),systolic velocity(Vs),and diastolic velocity(Vd)of the middle cerebral artery in the treatment group and the control group were higher than those before treatment,and pulsatility index(PI)and resistant index(RI)were lower than those before treatment,and the difference was statistically significant(P<0.01).After treatment,the difference of Vm,Vs,Vd between the two groups was statistically significant(P<0.05).The treatment group was superior to the control group,but there was no significant difference between the two groups after treatment with PI,RI(P>0.05).(2)Comparison of hemorheology:After treatment,the whole blood viscosity shear rate(1/S),whole blood viscosity shear rate(5/S),whole blood viscosity shear rate(30/S)and whole blood viscosity shear rate(200/S)in the treatment group and the control group were lower than those before the treatment.The difference was statistically significant(P<0.01).There was significant difference between the two groups after treatment(P<0.01 or P<0.05),and the treatment group was superior to the control group.After treatment,theplasma viscosity,hematocrit,whole blood low cut reduction viscosity and whole blood high cut reduction viscosity in treatment group and control group were significantly lower than those before treatment(P<0.01 or P<0.05).There was no significant difference between the two groups after treatment(P>0.05).(3)Comparison of NIHSS scores and efficacy evaluation:After treatment,the NIHSS scores of the treatment group and the control group were decreased,and the difference was significant(P<0.01).The difference between the two groups was significant after treatment(P<0.01),and the treatment group was superior to the control group.The total effective rate was 86.7% in the treatment group and 63.3% in the control group.The total effective rate was compared between the two groups,and the difference was statistically significant(P<0.05).Conclusion:(1)Both Xingnao Tongdu acupuncture and traditional acupuncture can improve cerebral hemodynamics in patients with ischemic stroke(LAA type).In terms of increasing Vm,Vs and Vd of the middle cerebral artery in the lesion side,the Xingnao Tongdu acupuncture is superior to the traditional acupuncture.In reducing the PI and RI,the reduction degree of the two groups is equivalent.(2)Xingnao Tongdu acupuncture and traditional acupuncture can reduce the hemorheology of patients with ischemic stroke(LAA type).In reducing the viscosity of whole blood,Xingnao Tongdu acupuncture is better than traditional acupuncture.There was no significant difference between the two groups in reducing plasma viscosity,hematocrit,whole blood low cut reduction viscosity,and whole blood high cut reduction viscosity.(3)Xingnao Tongdu acupuncture and traditional acupuncture can reduce the NIHSS scores of patients with ischemic stroke(LAA type),improve nerve function,and have good clinical efficacy.Compared with traditional acupuncture,the effect of Xingnao Tongdu acupuncture is more significant.
