Clinical Evaluation Of Compound Gegen Qinlian Decoction In The Treatment Of Type 2 Diabetes Mellitus Complicated With Non-Alcoholic Fatty Liver Disease And Its Correlation With The Levels Of Chemerin And Resistin

Objective: The changes of TCM clinical symptoms and related test indexes of Compound Gegen Qinlian Decoction in treating type 2 diabetes mellitus(T2DM)combined with non-alcoholic fatty liver disease(NAFLD)will be objected.The influence on the level of serum Chemerin and resistin will be analyzed.The safety and efficacy of Compound Gegen Qinlian Decoction in treating this disease will be objectively evaluated and analyzed.And the efficacy of Compound Gegen Qinlian Decoction in treating T2 DM combined with NAFLD will be preliminarily explored.The mechanism of NAFLD will provide theoretical basis for clinical application.Methods: Sixty patients with T2 DM combined with NAFLD,corresponding to diagnosis of spleen deficiency and phlegm turbidity syndrome,were divided into treatment group and control group with 30 cases in each group by random number table method.The control group was given basic treatment(health education,proper exercise,diet control and body weight;routine treatment of hypoglycemia,lipid regulation,hypotension),and the treatment group was given compound Gegen Qinlian Decoction except for the basic treatment given in control group for 12 weeks.The changes of TCM syndrome score,BMI,WHR,FPG,2hPG,HbA1 C,FINS,HOMA-IR,TC,TG,LDL-C,HDL-C,ALT,AST,GGT,B-mode ultrasonography and the level of Chemerin and resistin were observed before and after treatment,and the safety indexes were detected.Results:(1)The total effective rate was 96.67% in treatment group,which was significantly higher than the rate of 73.33% which in control group(P < 0.05).(2)After treatment,the scores of TCM symptoms in both groups were lower than those before treatment(P < 0.05),and the decrease value in treatment group was significantly higher than that in control group(P < 0.05).(3)After treatment,BMI and WHR of the two groups had no significant changes compared with those before treatment(P > 0.05).(4)After treatment,the levels of serum FPG,2hPG,HbA1 C,FINS and HOMA-IR in the two groups were significantly lower than those before treatment(P < 0.05).The levels of serum FPG,2hPG,HbA1 C,FINS and HOMA-IR in treatment group were significantly higher than those in control group(P < 0.05).(5)After treatment,the levels of serum TC,TG and LDL-C in both groups were lower than those before treatment(P < 0.05),and the level of HDL-C was higher than that before treatment(P < 0.05).The decrease of serum TC and increase of HDL-C in treatment group were higher than those in control group(P < 0.05).(6)After treatment,the levels of serum ALT,AST and GGT in both groups had no significant changes compared with those before treatment(P > 0.05).(7)After treatment,the B-mode ultrasonographic grading of liver in treatment group was significantly better than that before treatment(P < 0.05),while the B-mode ultrasonographic grading of liver in control group was not significantly better than that before treatment(P > 0.05).(8)After treatment,the levels of serum Chemerin and resistin in two groups were significantly lower than those before treatment(P < 0.05),and the levels of serum Chemerin and resistin in treatment group were significantly higher than those in control group(P < 0.05).(9)The incidence of adverse reactions in treatment group was lower than that in control group(P > 0.05),and the safety was higher.Conclusion: Compound Gegen Qinlian Decoction can improve the clinical symptoms of patients with T2 DM combined with NAFLD,reduce blood sugar and lipid,and improve insulin resistance(IR)and liver B-mode ultrasonography.Its mechanism may be related to its ability to regulate the expression of the disease-related factors,to reduce IR,to correct inflammation,to increase oxidative stress and to up-regulation of hepatotoxic cytokines,with fewer adverse reactions and obvious curative effect.
