Comparison And Analysis Of The Application Values In Measurement Of Nulliparous Women's Pelvic Hiatal Area By The Technology Of Omniview-VCI And Render In Diagnosis Of Urinary Incontinence

ObjectiveBy analysing and comparing the repeatability,consistency and time consuming situation of Omniview-VCI and Render in measuring the area of pelvic hiatal in nulliparous women,to recommend a simple,time-saving and reliable ultrasonic technology for clinical.And supplying a reliable reference value of pelvic hiatal area for the urinary incontinence diagnosis of nulliparous women besides.In addition,the risk factors of the occurrence of urinary incontinence in nulliparous women were analyzed in the study so that to provide scientific basis for clinical prevention of the occurrence of urinary incontinence for nulliparous group.MethodsNulliparous women who admitted by our department and met the inclusion criteria were collected in our study,dividing into two groups by the symptoms of urine leakageun uncontrollably.Collecting the detailed general information,including the age,height,weight,waist circumference,history of childhood enuresis,the family history of pelvic floor dysfunction,marriage status,smoking status,history of chronic cough and chronic constipation,the intensity of daily exercise,education level and the residence.Through univariate analysis and Logistic regression analysis,to evaluate the risk factors of the occurrence of urinary incontinence in nulliparous women.Analysing and comparing the consistency,repeatability and time consumption in measuring the area of pelvic hiatal by Omniview-VCI and Render in nulliparous women.Comparing the percentage of infundibular urethral and posterior urethral angle opening,the descending distance of the bladder neck,the rotation angle of the urethra and the area of pelvic hiatal between the experimental group and the control group.ROC curve analysis was performed about the pelvic hiatal area in nulliparous women with urinary incontinence.Results1.In this study,140 nulliparous women were included,36 cases with urinary incontinence and the all were judged mild urinary incontinence,which containing 29 cases of stress urinary incontinence,5 cases of urgency urinary incontinence and 2cases of mixed urinary incontinence.2.According univariate analysis and Logistic regression analysis,the waist circumference and history of childhood enuresis are the risk factors of urinary incontinence in nulliparous women.3.Omniview-VCI and Render both show excellent intra-operator and inter-operator repeatability of the measurement of the pelvic hiatal area in nulliparous women.And the two technologies has great consistency between each other,but Omniview-VCI takes less time than Render mode.4.Compared with normal nulliparous women,the bladder neck descent,urethra rotation angle and the percentage of urethral mouth funnel forming were increased in urinary incontinence group.The pelvic hiatal area of the nulliparous women with urinary incontinence was larger than that in the normal nulliparous women on maximum Valsalva manner.5.The critical diagnosis values of pelvic hiatal area under maximum valsalva of nulliparous women urinary incontinence by Omniview-VCI and Render were14.55cm2 and 14.65cm2 respectively,and the sensitivity is 91.7% and 88.9% while the specificity is 83.7% and 84.6% respectively.Conclusions1.The waist circumference and history of children enuresis are significant risk factors of urinary incontinence in nulliparous women.2.Omniview-VCI and Render both show excellent intra-observer and inter-observer repeatability and great consistency between each other of measuring the pelvic hiatal area in nulliparous women,but Omniview-VCI takes less time than Render mode.Therefore,Omniview-VCI is more recomended to be an excellent measurement technique of the pelvic hiatal area in nulliparous women.3.The critical diagnosis values of pelvic hiatal area on maximum Valsalva in nulliparous women with urinary incontinence by Omniview-VCI and Render were14.55cm2 and 14.65cm2 recomended respectively.
