ObjectiveTo understand stroke patients' hope level,the status quo of medication adherence in stroke risk factors;Analysis the correlation between hope level and medication compliance in stroke patients;To explore the population sociology datas and disease related information effects on the hope level in stroke patients;In order to provide a reference for medical workers in improving the level of hope and medication compliance of stroke patients,To provide theoretical basis for further continuing nursing and secondary prevention of stroke patients.MethodsIn this study,the inclusion and exclusion criteria were strictly followed,the convenience sampling method was adopted.From November 2017 to May 2018,the general information questionnaire,Herth Hope Index(HHI),8 items of Morisky medication compliance scale and the evaluation of patients with basic daily life activities of the Barthel Index(BI)were successively used.The questionnaire survey was conducted among 148 patients,all the respondents from a Third Grade A hospitals in ZhengZhou Henan province,the follow-up personnel adopted the unified guideline.A month after discharge,patients were surveyed by telephone about their medication situation.After this,two researchers used Epidata3.1 to input the relevant data collected,and used the statistical software SPSS19.0 to sort out and analyze the data,and described the number and proportion of population sociology and disease data.Patients with cerebral apoplexy were expected to use mean and standard deviation to describe their level and medication compliance.Demographic sociology and disease data were described by case number and proportion;Mean and standard deviation was used to describe the hope level and medication compliance of stroke patients;T-test and variance test were used to describe the impact of patients' demographic and sociological grouping of disease-related data on their level of hope and medication compliance;Pearson was used to test the relationship between the level of hope and drug compliance in patients with stroke;All data with statistically significant differences,level of hope,and each dimension of hope level as an independent variable.Multiple linear regression analysis was used to determine the main influencing factors of drug compliance in stroke patients.The test standard is ?= 0.05.Results1.Stroke patients with general data,strict according to include and exclusion criteria,the investigation questionnaires out of 150,148 effective questionnaires were taken back(2 midway asked to quit because of patients),effective recovery rate was 98.7%.The object of study,83 cases of male patients accounted for 56.1%,65 cases of female patients accounted for 43.9%,the patients' average age was(58.70 ±13.51);With 120 cases of ischemic cerebral stroke patients(81.1%),28 patients with hemorrhagic cerebral stroke was 18.9%;69 patients are living in the city accounted for 46.6%,living in the countryside 79 patients accounted for 53.4%and other relevant information.2.The BI score of the patients' daily activities showed that there were 26 patients without dependence,accounting for 17.6%;93 patients with mild dependence,accounting for 62.8%;22 patients with moderate dependence,accounting for 14.9%;and 7 patients with severe dependence,accounting for 4.7%.3.The total score of hope level for the patients with cerebral stroke is(33.14±4.46)points,hope level is at the medium level,in 148 patients,three of which 2.0% of respondents hope level is low,located in the middle level of patients with 111(75.0%),34 cases accounted for more than 23.0% of respondents hope to higher levels;The positive action dimension was(11.12±1.91).The total dimensions of maintaining intimate relationship with others was(11.06 ±1.75).The dimensions of positive attitude towards reality and the future was(10.96±1.72)points.4.The total score of medication compliance of stroke patients was(4.71±2.00),poor medication compliance in overall patients,and 106 patients with poor drug compliance accounted for 71.6%;42 patients with good compliance accounted for 28.4%;Among the 8 items medication adherence scale,the three items with the lowest average score were: whether you found it difficult to stick to the medication plan was(0.27 ±0.45),Do you sometimes forget that your medication score is(0.35±0.48);When your disease condition is under control,the score for whether you have stopped taking the medication on your own is(0.49±0.50)points.5.The general data of stroke patients and BI score were used for univariate analysis of the score of hope level and medication compliance,showing that there were differences and statistical significance in the living environment,level of education,current working status,BI score,understanding degree of the disease and the level of hope were different and statistically significant(P<0.05).The medical insurance type,smoking status,drinking status,stroke type,BI score,whether the patient had first onset,the degree of understanding of the disease,the number of sequelae,days in hospital and the score of medication compliance were different and statistically significant(P<0.05).6.Patients with cerebral stroke hope level and three dimension scores are positively correlated with medication compliance(P < 0.05).7.In multiple linear regression analysis,the total score of cerebral stroke patients medication compliance as a dependent variable,cerebral apoplexy patients want to level the total score and each dimension score and single factor analysis was statistically significant of the relevant materials as independent variables,including health care type,smoking status,the present state of drinking,parting,Barthel index,stroke is the first time,degree of illness to understand disease,sequela of species,the length of days,and after giving order classification variables to convert multiple linear regression analysis,the analysis results show that the type of stroke,whether it is the first time onset,sequela species,length of hospital stay,positive attitude towards the reality and the future of five variables into the regression equation,and the five variables can explain 40.0% of the total variance.Conclutions1.Hope level in stroke patients is at a moderate level,medication adherence is relatively poor.2.The higher hope level,the better the medication compliance.The lower hope level,the worse medication compliance;There was a positive correlation between the level of hope and drug compliance in stroke patients.3.The living environment,level of education,current working status,BI score and the understanding of disease condition are the influencing factors of hope level of stroke patients.4.The types of stroke,whether it is the first time onset,the types of sequelae,the number of days in hospital,and the positive attitude towards the reality and future are the most important factors affecting the medication compliance of stroke patients.
