The Correlation Study Of The Disease Uncertainty,Positive Feelings And Hope Level Of Stroke Patients' Caregiver

Objective:Exploring current situation of the main stroke patients' family caregiver in uncertainty illness,positive feelings and hope level;analysing influence factors of disease uncertainty;studying the correlation of disease uncertainty,positive feelings and hope level in the stroke patients' family caregiver,so as to provide some reference for helping family members to reduce the indeterminate sense of disease,improve disease cognition,enhance caring positive feelings and hope level.Method:This study selected two tertiary general hospitals in Hunan province,and then adopt 211 cases of neurology hospital stroke patients' families caregivers in two tertiary hospital from June 2017 to February 2018 as the research object.A questionnaire survey was conducted among those family members of the patients,including questionnaire for general data of patients and family members,Barthel Index Scale,National Institute of Health Stroke Scale,modified Rankin Scale,Mishel Uncertainty in Illness Scale for Family Member,Positive Aspects of Caregiver Scale,Herth Hope Index scale.The SPSS 20.0 was used to data analysis and process,the statistical methods had statistical description,Spearman correlation analysis,t test,variance analysis,LSD comparison,kruskal-wallis H test,and multivariate linear stepwise regression analysis.Results:?1?The indeterminate sense of disease of the 211 family caregivers of cerebral stroke patients' total score was?86.90 ±10.15?points,each dimension of average score from high to low was unpredictable?3.14±0.86?,uncertainty?3.00±0.95?,complexity?2.82 ±0.95?and the lack of information,?2.63±1.92?points.?2?The total score of the 211 family caregivers of cerebral stroke patients was?29.14±1.69?points,in which the self affirmation dimension was?3.63±0.53?points,and the life outlook dimension is?2.74±0.72?points.?3?The total score of hope level of the 211 family caregivers of cerebral stroke patients was?36.68±1.65?points,and the positive attitude towards reality and future was?3.01±0.43?points,the taking positive action was?3.11±0.33?points,and keeping close relationship with others was?3.05 ±0.29?points.?4?The single factor analysis of variance showed that the factors of family caregivers such as age,educational background,occupations,family per monthly income,residence,physical condition had different effects on the indeterminate sense of disease of the family caregivers of stroke patients,the difference had statistically significant?P<0.05?;In the general information of the patients,the factors such as the number of stroke attack,duration of illness,whether there was a combination of other diseases,duration of disease,days of hospitalization,the way to pay for medical expenses and the self-care ability,NIHSS score of patients had impact on the indeterminate sense of disease of the family caregivers of cerebral stroke patients.The difference had the statistically significant?P<0.05?;Multiple linear stepwise regression analysis showed that patients' hospitalization days?X₆?,patients' daily life ability?X₃?,family per capita income?X₉?,self esteem?X₁2?,positive feelings?X₁4?,hope level?X₁8?into the stepwise regression equation.?5?Spearman correlation analysis showed that the indeterminate sense of disease of the family caregivers of cerebral stroke patients was negatively correlated with thepositive feelings of care?P<0.05?,the indeterminate sense of disease was negatively correlated with the level of hope?P<0.05?,and the level of hope was positively correlated with positive feelings.?P<0.05?.Conclusions:?1?The uncertainty in illness and positive feelings of the cerebral stroke patients'family caregivers were at the medium level.?2?The patient whose hope level was at medium to high status has high hope for treatment.?3?The hospitalization days of patients,patients' dailylife ability,family members of the family per capita income level,self esteem and positive feelings,hope was family caregivers stroke disease uncertainty factors.?4?The disease uncertainty of the stroke patients' family caregiver was negatively correlated with positive feelings and hope level;While positive feeling is positively correlated with hope level.
