Background:Primary hypertension(PH),also known as hypertensive disease,is the most common cardiovascular disease in China.It is a systemic disease characterized by elevated systemic arterial pressure.It is one of the major chronic diseases that currently threaten human health and is the most important risk factor for cardiovascular and cerebrovascular diseases.The latest data show that the prevalence of hypertension in adults is 25.2%,and the estimated number is 270 million.Cardiovascular and cerebrovascular complications caused by hypertension seriously affect people's quality of life and is one of the leading causes of death and disability in patients with hypertension.The study found that chronic inflammatory response is involved in the formation of hypertensive vasculopathy and plays a decisive role in the occurrence and development of hypertensive angiopathy.The carotid artery acts as a channel connecting the two important organs of the heart and brain.The lesions are often earlier than the coronary arteries and are regarded as the observation window of systemic arteriosclerotic lesions.Clinically,the blood vessels can be observed by surface ultrasonography.In recent years,Platelet-to-lymphocyte ratio(PLR)as an emerging indicator of inflammation,is cheap and easy to obtain,and has become a research hotspot in various disciplines.A number of studies have also confirmed that PLR is associated with the prognosis of acute coronary syndrome,stroke,type 2 diabetes,peripheral arterial disease,and malignant tumors.Objective:To study the relationship between platelet-to-lymphocyte ratio(PLR)and carotid atherosclerosis in patients with primary hypertension.Method:A case-control study included 309 patients with primary hypertension(PH)and146 normal control(NC).309 hypertension patients were classified into two groups,the primary hypertension without carotid atherosclerosis(PH+NAS,n=157)and primary hypertension with carotid atherosclerosis(PH+AS,n=152).The clinical and laboratory markers were collected in each groups.NLR,PLR and laboratory markers were compared in different groups.Logistic regression analysis was used to analyze the influencing factors of carotid atherosclerosis in patients with primary hypertension.Result:1.The age,SBP,DBP,FBG,TG,Cr,UA,TC,LDL-C,WBC,N,Plt,NLR and PLR were significantly higher in the PH group(p<0.05),and the level of HDL-C was lower than the NC group,the difference was statistically significant(p<0.05).2.The age,smoking rate,cerebrovascular accident prevalence,SBP,FBG,BUN,NLR and PLR levels were higher than the PH+NAS group and the difference was statistically significant(p<0.05).3.Multivariate logistic regression analysis showed that age,FBG,smoking,SBP,and PLR were the independent risk factors for carotid atherosclerosis in patients with primary hypertension.Conclusion:The level of PLR are associated with Primary hypertension.Multivariate logistic regression analysis indicated that PLR may be an independent risk factor for carotid atherosclerosis in patients with primary hypertension.
