Carotid stenosis is an important arterial occlusive disease that can lead to ischemic cerebral infarction and stroke.Atherosclerosis plays a role in 90% of all extracranial carotid diseases.Atherosclerosis is a chronic inflammatory disease affecting systemic arteries.Inflammatory status is associated with poor prognosis in atherosclerosis.Some circulating inflammatory proteins,such as C-reactive protein or interleukin-6(IL-6),have been identified as reliable biomarkers of carotid atherosclerosis,and the significance of leukocytes as predictive markers in some cardiovascular diseases has been pointed out,including neutrophil to lymphocyte ratio(NLR)and platelet to lymphocyte ratio(PLR).The roles of NLR and PLR in coronary artery disease,myocardial infarction,aortic aneurysm,and macrovascular surgery have been studied.The purpose of this thesis is to discuss the correlation between PLR and NLR in the degree of carotid stenosis.Objectives:The laboratory test results and clinical basic data of the patients after admission were analyzed retrospectively.The degree of carotid stenosis was obtained by computed tomography angiography(CTA),the correlation between the ratio of neutrophil to lymphocyte(NLR),platelet to lymphocyte ratio(PLR),and the degree of carotid stenosis was evaluated,and the risk factors of carotid stenosis were discussed.Methods:236 patients who were hospitalized in our hospital from January 2017 to June 2019 and met the inclusion criteria for suspected carotid stenosis undergoing carotid CTA were randomly selected as the subjects of this study.According to the results of carotid CTA,173 patients were divided into two groups: 105 males and 68 females in the carotid stenosis group,with an average age of(61.33±9.28)years,and 63 patients in the non-carotid stenosis group,including 31 males and 32 females.The average age was(52.73±9.20)years.The clinical data and basic data of the study subjects were collected,and Pulse pressure(PP),body mass index(BMI),PLR and NLR were calculated.The stenosis and degree of carotid artery were examined by carotid CTA,and the imaging results were diagnosed by two doctors.Reduce statistical bias.Results:1 Comparison of general data between the experimental group and the control group: compared with the group without carotid stenosis,the age of the carotid stenosis group was(52.73±9.21)vs(61.33±9.28),smokers were 44.4%(28/63)vs 58.9%(102/173),blood glucose was(6.08±1.70)vs(7.86 3.40),high density lipoprotein was(1.15±0.31)vs(1.03±0.33),systolic blood pressure was(129.38±17)vs(150.43±21.81),The differences were statistically significant(all P<0.05).Compared with the group without carotid stenosis,In the carotid stenosis group,the sex ratio was(31/32)vs(105/68),the average platelet volume was(10.41 ± 1.03)vs(10.20 ± 1.16),the triglyceride ratio was(1.65 ± 0.79)vs(1.98 ± 1.26),the low density lipoprotein ratio was(2.14±0.59)vs(2.25±1.04),the heart rate was(74.83±10.64)vs(74.72±11.27),and the BMI was(25.00±2.92)vs(25.31±3.92),the difference was not statistically significant(all P>0.05).2 Risk factors for carotid stenosis Logistic regression model analysis,single factor screening,through the above with P < 0.05 variables into the model fitting,into qualified risk factors,the results showed that age,smoking,abnormal blood glucose,higher systolic pressure,diastolic blood pressure,pulse pressure difference bigger,PLR,NLR is a risk factor for carotid stenosis,and high density lipoprotein is carotid stenosis degree protection factors.Logistic regression risk factor model of carotid artery stenosis analyzed the risk as follows: age OR value was 1.102(95%CI: 1.064,1.141),P=0.000<0.05;The OR value of smoking was 1.769(95%CI: 1.003,3.214),P=0.049<0.05.The blood glucose OR value was 1.370(95%CI: 1.161,1.616),P=0.000<0.05.The OR value of HDL was 0.345(95%CI: 0.148,0.805),P=0.014<0.05.The OR value of systolic blood pressure was 1.056(95%CI: 1.037,1.075),P=0.000<0.05.Diastolic blood pressure OR value was 1.102(95%CI: 1.064,1.141),P=0.000<0.05.The OR value of pulse pressure difference was 1.073(95%CI: 1.046,1.101),P=0.000<0.05.The OR value of PLR was 1.007(95%CI: 1.001,1.013),P=0.029<0.05.The OR value of NLR was 1.257(95%CI: 1.004,1.573),P=0.045<0.05.3 Correlation analysis between the levels of NLR and PLR and the degree of carotid stenosis: the levels of PLR and NLR in the carotid stenosis group were positively correlated with the degree of carotid stenosis(r=0.253 P <0.01;R =0.372 P <0.01).4 PLR,NLR,the predictive value of carotid stenosis: carotid artery of severe stenosis and occlusion of PLR,NLR ROC curve analysis and calculate the area under the curve(motorcycle aunder the curve,AUC)and PLR,NLR has predictive value(P<0.05),the PLR AUC 0.613(95% CI: 0.519-0.707),the best capture a value of 128.41,sensitivity of 63%,specificity of 58.7%,NLR AUC was 0.616(95% CI: 0.521-0.710),the best interception value is 2.76,sensitivity is 52.1%,specificity is 74.6%.Conclusions:1 Age,smoking,blood glucose,systolic blood pressure,diastolic blood pressure,pulse pressure difference,PLR and NLR are risk factors for carotid stenosis.2 PLR and NLR are positively correlated with the degree of carotid artery stenosis,among which NLR may be more useful than PLR in predicting the degree of carotid artery stenosis,and can be a convenient and inexpensive detection method,as a supplement to imaging examination,which can be used for clinical reference. |
