BackgroundEpilepsy is a chronic brain disease caused by multiple causes.The prevalence of epilepsy in China is between 4 ‰ and 7 ‰,especially among children and adolescents.The corpus callosum is a palliative operation in epilepsy surgery.The corpus callosum blocks the connection between the brain and limits the discharge of epilepsy to one side of the cerebral hemisphere,thus reducing or even ending the seizure.The application of corpus callosum in adult refractory epilepsy has been widely reported,but rarely in children.For refractory epilepsy children,not only to control seizures,but more importantly,to consider the development of brain function This stydy aims to investigate the clinical effects of corpus callosum in children with refractory epilepsy and the effect of postoperative on cognitive function in children.ObjectiveTo summarize the clinical effect of corpus callosum in children with refractory epilepsy,observe the frequency and severity of seizures in children after surgery,and pay attention to the improvement of intelligence and cognitive function of postoperative children.To evaluate the effect of corpus callosum on the efficacy and cognitive function of children with refractory epilepsy.Promote the understanding of the treatment of refractory epilepsy and guide the clinical treatment of refractory epilepsy in children.MethodsFrom April 2016 to August 2018,47 children with intractable epilepsy were treated in pediatric,neurosurgical ward of 988 Hospital of PLA.They were divided into operation group(25 cases)and control group(22 cases)according to the treatment methods.The operation group received corpus callosotomy and continued to receive antiepileptic drugs after operation,while the control group only received antiepileptic drugs.Six months later,the effect was evaluated according to the change of seizure frequency before and after treatment,and the cognitive function of the children was evaluated by Wechsler Child Intelligence Scale before and 6 months after treatment.Results(1)Six months after operation,the overall effective rate(92.00%)of the operation group was significantly higher than that of the control group(63.64%),with statistical significance(P < 0.05);(2)Compared with the preoperative,the IQ,operation score and speech score of the operation group were significantly improved,with statistical significance(P < 0.05).(3)In the operation group,2 patients(8%)developed transient dissociation syndrome.2 patients(8%)had subcutaneous effusion after operation.The effusion absorbed after 1 week.There were no adverse drug reactions in the two groups during the treatment period,no other complications such as long-term hemiplegia and aphasia,no deaths.Conclusion1.Corpus callosotomy is safe and effective in the treatment of children with intractable epilepsy.2.Corpus callosotomy improved the IQ,speech score,and operation scores of cognitive function in children with refractory epilepsy.
