ObjectiveTo explore the value of spectral CT imaging in the muti-parameter analysis of hypovascular clear cell renal carcinoma and chromophobe cell renal carcinoma.MethodsA retrospective analysis was performed on 52 patients with renal tumor, which was proved pathologically including 35 cases with CCRCC and 17 cases with CCRC were scanned in GSI mode.GSI Viewer software was used to calculate the different parameters of GSI in the cortical phase(CP)and medullary phase(MP).The region of the masses in 70keV monochromatic energy CT value, iodine concentration(IC),and then standardize them. The CT spectral curves were calculated in the CP and MP.Those parameters was evaluated statistically between hypovascular CCRCC and CCRC by two-simple t testResultsIn the CP,the differences between the CCRCC and CCRC groups were found in corresponding CT values(76.51±9.24)HU and(73.90±8.37)HU(P>0.05),CT value ratios(0.54±0.08)and(0.49±0.09)(P<0.05)at 70keV monochromatic energy, the IC(22.09±4.11)and(19.97±4.01)(P>0.05),the normalized iodine concentration(NIC)(0.37±0.09)and(0.32±0.06)(P<0.05),the slope rates of spectrum energy energy curves(2.85±0.72)and(2.42±0.48)(P<0.05).Similarly,in the NP,the differences between the CCRCC and CCRC groups were found in corresponding CT values(95.22±13.01)HU and(84.40±9.33)HU,CTvalue ratios(0.65±0.15)HU and(0.55±0.07)HU at 70keV monochromatic energy, the IC(33.23±6.96)and(22.94±4.46),the NIC(0.64±0.19)and(0.46±0.11),the slope rates ofspectrum energy energy curves(3.17±0.72)and(2.69±0.47)respectively(P<0.05).The IC in the NP provided high sensitivity in differentiating CCRCC from CCRC(72%and 99%,respectively),and the threshold value is 29.730,the area under the ROC curve is 0.909;In addition, the study found that there were 5 cases of 17 cases of CCRC in the group, which accounted for 29%of the study.ConclusionThe imaging findings of conventional spiral CT combined with multi-parameter analysis of GSI have certain diagnostic value in the differential diagnosis of renal blood deficiency for clear cell renal carcinoma and chromophobe cell renal carcinoma.
